Background and Aims: Stroke is a leading cause of seizures and epilepsy in adults, but current guidelines lack strong recommendations for treating post-stroke seizures (PSE). This study aims to demonstrate the safety and efficacy of lacosamide combined with non-vitamin K antagonist oral anticoagulants (NOACs) in patients with PSE and atrial fibrillation (AF). Methods: In this prospective longitudinal study, 53 patients with PSE and AF, admitted from 2022 to 2023, received NOACs for AF management and lacosamide for seizure control. A control group of 53 patients with cardioembolic stroke, receiving NOACs, was matched by age, sex, and NIHSS scores to ensure comparability. Results: Over 24 months, 16 patients in the study group and 15 in the control group experienced new embolic events, with no significant difference between groups. Seizure control improved significantly in the study group, with reduced frequency and severity. No severe adverse effects from lacosamide were observed. Conclusions: The combination of NOACs and lacosamide is a safe and effective treatment for patients with AF and PSE, not increasing the risk of recurrent ischemic or hemorrhagic events. Further studies with larger sample sizes and longer follow-ups are needed to confirm these findings and optimize treatment protocols.

The collection of photo shoots during the systematic monitoring activities is useful to witness the ecological role of marine phanerogams as hosts for a rich variety of organisms in coastal and transitional waters. Cymodocea nodosa is present in the Taranto seas. In the Mar Piccolo, it reached high covers in short times, up to 100%, due to the bettering of the environmental conditions. The most recent observations showed that it offers welcoming towards several vertebrates and invertebrates, native and non-indigenous, as well as to micro and macroalgae. The NPPR funded activities will make these observations more robust and structural.

Previous research has already examined the relationship between Future Anxiety, a construct recently introduced in Italy, and mental health in young adults, although possible mediating variables in this relationship have so far never been investigated. The present study attempts to fill this gap by exploring the incidence of Future Anxiety on psychological distress in a group of 302 young Italian adults (18 - 30 years; M = 21.9; SD = 2.6), presenting and evaluating the simultaneous mediating effect of Intolerance of Uncertainty and Non-Pathological Worry.Findings highlighted how Future Anxiety had a positive and significant direct effect on Stress and Depression, but not on Anxiety. In the three serial mediation models proposed, Intolerance of Uncertainty and Non-Pathological Worry mediated the relationship between Future Anxiety and mental health outcomes. The results also confirmed the hypothesized serial mediation effect by highlighting how young adults with greater Future Anxiety experienced more Intolerance of Uncertainty, which positively affected Non-Pathological Worry levels and, in turn, exacerbated psychological distress. Finally, results indicated that female participants experienced more Stress, Anxiety, and Depression in relation to Future Anxiety compared to males. Starting from the review of main references on this subject, the results discussed provide new insights for understanding youth psychological distress. Finally, practical implications for the design of supportive interventions for this study’s target group are proposed.

Sublobar resection is a standard surgical procedure for small-sized non-small cell lung cancer (NSCLC). However, clinical role of adjuvant chemotherapy for small-sized NSCLC with pathological lymph node (LN) metastasis after sublobar resection is unknown. National Cancer Database was queried for NSCLC patients between 2004 and 2018. Eligibility included sublobar resection with pathological LN metastasis, R0 resection, Charlson comorbidity score = 0, clinical stage T1a-b, and tumor size ≤ 20 mm. Kaplan-Meier method with log-rank test and multivariable Cox proportional hazards analyses were used for assessing survival. Samples were evaluated before and after propensity score matching (PSM) with respect to age, sex, histologic type, and pathological LN status. Of 810 patients who met the eligibility criteria, 567 (70.0%) underwent adjuvant chemotherapy. After PSM, patients with adjuvant chemotherapy had a significantly longer survival than those without (median survival: 64.3 vs. 34.0 months, hazard ratio for death: 0.61, p < 0.0001). Multivariate analyses after PSM showed that younger age (p = 0.0206), female (p = 0.0005), and adjuvant chemotherapy (p < 0.0001) were independent prognostic factors for longer survival. Adjuvant chemotherapy has a prognostic impact in patients with small-sized NSCLC and pathological lymph node metastasis who undergo sublobar resection.

Spatial and temporal variation of the free electron concentration in the ionosphere affects SAR interferograms, in particular at low radar frequencies. In this work the identification, estimation and compensation of ionospheric path delay phases in PALSAR-3 and NISAR-L interferograms is discussed. Both these L-band sensors acquire simultaneously SAR data in a main spectral band and in an additional, spectrally separated, narrower second band to support the mitigation of ionospheric path delays. The methods presented permit separating the dispersive and the non-dispersive phase terms based on the double-difference interferogram between the two available spectral bands and the differential interferogram of the main band. The applicability of the proposed methods is demonstrated using PALSAR-3-like data that was simulated based on PALSAR-2 SM1 mode data.

Determining precise boundaries and protective zones for historical and cultural objects enables their effective preservation. This is particularly crucial in the face of increasing tourism, urbanization, and natural disasters. Accurately defining boundaries and protective zones can aid in the planning of scientific research, archaeological excavations, and other studies aimed at exploring the history and cultural significance of these objects. The article presents the framework for establishing protective zones around historical and cultural heritage sites existing in Ukraine, using the example of the Citadel defensive complex located in Lviv (Ukraine). It proposes general and detailed conceptual models for the combined application of remote and non-invasive methods for investigating historical and cultural heritage sites. This conceptual article introduces the theory of integrating radar satellite imaging with ground-based georadar imaging. Additionally, it presents a software module that has been developed to analyze collected data on two-dimensional historical and cultural heritage objects, refine their boundaries, and establish protective zones around them.

This study describes the implementation of IoT sensors in drinking water systems and fog collector systems in low-income communities in Ecuador. The influence and handling of these sensors are analyzed, as well as the materials and methods used in their implementation. The importance of validating the accuracy and reliability of IoT sensors compared to professional devices, especially in mountain areas, is highlighted. In addition, it is mentioned that the cost-benefit of using IoT sensors in fog catchers and drinking water networks depends on several factors, such as the scale of the project, specific objectives, and available resources. Finally, it is highlighted that the use of IoT sensors in smart construction and water collection systems has proven to be beneficial in preventing effects on the operation.

The aim of this randomized controlled clinical trial was to compare the 2-year clinical performance of resin composite restorations placed at non-caries cervical lesions (NCCL) with one-step self-etch, total-etch, and selective enamel etch & self-etch adhesive techniques. Thirty-two patients received three resin composite restorations each at NCCLs (Tetric EvoCeram/Ivoclar/Vivadent), bonded with a total-etch adhesive agent (ExciTE F/Ivoclar/Vivadent) and a self-etch (AdheSE One F/Ivoclar/Vivadent) without and with selective enamel etching. All restorations were evaluated by two examiners at baseline, 6-, 12-, 18- and 24-months post-operation regarding retention, caries occurrence, marginal adaptation, and marginal staining. A logistic regression analysis with generalized estimating equations was used. After 2 years, the retention rate was 86,8% for total-etch, 92.26% for self-etch, and 93.63% for selective enamel etching & self-etch. No caries were detected during all the restorations. Concerning marginal adaptation, the clinically perfect restorations were 26.9% for the total-etch technique, 16% for self-etch, and 25,9% for selective enamel etch & self-etch. All three adhesive strategies provided restorations with no significant differences in the retention rate and the marginal adaptation, whereas the total etch yielded better performance for marginal staining. All the restorations were assessed as clinically acceptable after 2 years.

Bladder cancer (BC) possesses distinct molecular profiles that influence progression depending on its biological nature and delivered treatment intensity. Muscle-invasive BC (MIBC) and non-MIBC (NMIBC) demonstrate great intrinsic heterogeneity regarding different prognoses, survival, progression, and treatment outcomes. Transurethral resection of bladder tumor (TURBT) is the standard of care in treating NMIBC and serves both diagnostic and therapeutic purposes, despite the prevalent recurrence and progression among many patients. In particular, flat urothelial car-cinoma in situ and urothelial carcinoma with lamina propria invasion are the major precursors of MIBC. A new‐generation photosensitizer, 5‐Aminolevulinic acid (5-ALA), demonstrates high tumor specificity by illuminating the tumor lesion with a specific wavelength of light to produce fluorescence, and has been studied for photodynamic diagnosis to detect precise tumor areas by TURBT. Additionally, it has been applied for treatment by producing its cytotoxic reactive oxygen species, as well as screening for urological carcinomas by excreting porphyrin in the blood and urine. Moreover, 5-ALA may contribute to screening before and after TURBT in NMIBC. Here, we summarize the updated evidence and ongoing research on photodynamic technology for NMIBC, providing insight into the potential for improving patient outcomes.

Acoustic emission monitoring (AEM) has emerged as an effective technique for detecting wire-breaks resulting from, e.g., stress corrosion cracking, and its application on prestressed concrete bridges is increasing. The success of this monitoring measure depends crucially on a carefully designed sensor layout. For this the attenuation of elastic waves within the structure’s material is determined ideally in-situ through object-related measurements with a reproducible signal source, typically a rebound-hammer. This assumes that the attenuation coefficients derived from rebound-hammer tests are comparable to those from wire-breaks, thus allowing their results to be directly applied to wire-break detection without further adjustments. This study challenges this assumption by analysing attenuation behaviour through an extensive dataset. Employing time-domain and frequency analysis, the research generates attenuation profiles from laboratory experiments and in-situ measurements across various girders and bridge structures, extracting the slope and standard deviation of the residuals. While generally validating this approach, the findings highlight differences in attenuation behaviour from among wire-break signals and rebound-hammer impulses, whereby the latter potentially underestimates the relevant attenuation of wire-breaks by approximately 20 %. Consequently, a transfer factor is proposed to adjust object-related measurement results for wire-break scenarios, including a variance of 1.0 dB/m to cover a 95 % confidence interval for sample scattering. Moreover, the anisotropic attenuation behaviour across different structures was studied, showing that transverse attenuation consistently exceeds the longitudinal, significantly influenced by structural features such as voids. In prefabricated concrete bridges with in-situ-cast concrete slabs, transverse signal transmission remains unhindered across multiple elements. Finally, the results provide a valuable reference for the design of sensor layouts in bridge monitoring, particularly benefiting scenarios where direct in-situ experiences are lacking.

Recent research has implicated the gut microbiota in the development of lymphoma. Dysbiosis of the gut microbial community can disrupt the production of gut microbial metabolites, thereby impacting host physiology and potentially contributing to lymphoma. Dysbiosis-driven release of gut microbial metabolites such as lipopolysaccharides can promote chronic inflammation, potentially elevating the risk of lymphoma. In contrast, gut microbial metabolites, such as short-chain fatty acids, have shown promise in preclinical studies by promoting regulatory T-cell function, suppressing inflammation, and preventing lymphoma. Another metabolite, urolithin A, exhibited immunomodulatory and antiproliferative properties against lymphoma cell lines in vitro. While research on the role of gut microbial metabolites in lymphoma is limited, this article emphasizes the need to comprehend their significance, including therapeutic applications, molecular mechanisms of action, and interactions with standard chemotherapies. The article also suggests promising directions for future research in this emerging lymphoma and gut microbiome investigation field.

Background: Feline infectious peritonitis (FIP) is a fatal feline disease classified as either effusive (‘wet’), non-effusive (‘dry’), or a mixture of both forms (‘mixed’). The anti-FIP therapeutic effect of Mutian and molnupiravir, two drugs whose active ingredient is a nucleic acid analog, has been confirmed recently. Methods: Of the cats with FIP, we observed a total of 122 and 56 cases achieved remission after administration of Mutian and molnupiravir as routine treatments, respectively. Changes in clinical indicators suggested to be correlated with FIP remission (weight, hematocrit, and albumin to globulin ratio) before and after the administration of each drug and during follow-up observation were statistically compared for each FIP type. Results: In all three FIP types, administration of either Mutian or molnupiravir resulted in statistically significant increases in these indicators. Furthermore, the effect of Mutian on improving the albumin to globulin ratio was not observed at all in wet FIP, as compared with that of molnupiravir, but statistically significant in mixed and dry (P<0.02 and P<0.003, respectively). These clinical features were all due to differences in fluctuations of circulating globulin levels. Conclusions: These results indicate slight inflammatory responses might be elicited continuously by residual feline coronavirus non-disappeared through molnupiravir treatments.

Photovoltaic materials convert light into electrical voltages. So far, most research in the field are focused on how to refine/improve the existing or standard photovoltaic cells to yield more efficient electricity, to reduce weight, to make more flexible shape, or to minimize costs under standard and uniform sunlight illuminations. In this project, we explore and demonstrate a hypothesis that non-uniform light illumination photovoltages can also be generated due to photo doping induced mobile charge carrier density gradient between light and dark areas in either p-type or n-type photo doping semiconductors. The term non-uniform light illumination was coined to refer to subjecting our photo sensing thin film devices to areas of high and low intensities of light illumination simultaneously, with an intention of creating a charge density gradient and therefore a voltage. Specifically, a polymeric composite thin film composed of P3HT (p-type photo doping semiconductor) and PCBM (photo doping electron acceptor and trap) at varying p-doping levels were studied. We observed that the fabricated photovoltaic cells with about 80% PCBM doped P3HT thin film devices exhibited 0.5-1.2 volts photovoltages under non-uniform light illuminations.

We explores the hydrodynamical limit of a homogeneous electron gas, considering the interplay between the infrared limit and phenomenological hydrodynamics. Our study involves interactions generated by non-linear coordinate transformations. The focus begins with an analysis of non-linear coordinate transformations and their impact on normal modes in a harmonic system. The Lagrangian for a harmonic oscillator is used as an illustrative example, highlighting the role of position-dependent effective mass in preventing certain coordinate values. Moving beyond single degrees of freedom, we extend the discussion to a system with several harmonic degrees of freedom, introducing a collective variable. The effective dynamics of this collective variable are defined through a Lagrange multiplier term, emphasizing linearity in the description. Our study extends to the Current Two-Point function (CTP) formalism, introducing matrices and their inverses, such as $\hD$ and $\hD^{-1}$. Further, we delve into the dynamics of current in a homogeneous electron gas at finite density and zero temperature. Green's functions are generated using a functional integral approach, and the effective action is constructed via functional Legendre transformation. The quadratic part of the generator functional is expanded to derive the effective action, leading to the Maxwell equation and the subsequent elimination of the photon field. The resulting effective action for current dynamics is expressed in terms of the Current Two-Point function, paving the way for further exploration of the hydrodynamical aspects of the electron gas. Our study combines theoretical developments and mathematical formalism to provide insights into the hydrodynamical behavior of homogeneous electron gases.

Prokaryotic genomes are dynamic tapestries that are strongly influenced by mobile genetic elements (MGEs), including transposons (Tn’s), plasmids, and bacteriophages. Of these, miniature inverted-repeat transposable elements (MITEs) are undoubtedly the least studied MGEs in bacteria and archaea. This review explores the diversity and distribution of MITEs in prokaryotes and describes what is known about their functional roles in the host and involvement in genomic plasticity and evolution.

Single-particle inductively coupled plasma mass spectrometry (spICP-MS) has been used for particle size measurement of diverse types of individual nanoparticles and micrometer-sized carbon-based particles, such as microplastics. However, its applicability to the measurement of micrometer-sized non-carbon-based particles such as silica (SiO2) is unclear. In this study, the applicability of spICP-MS to particle size measurement of non-porous/mesoporous SiO2 micro-spheres with a nominal diameter of 5.0 µm or smaller was investigated. Particle sizes of these microspheres were measured using both spICP-MS based on a conventional calibration approach using an ion standard solution and scanning electron microscopy (SEM) as a reference technique and the results were compared. The particle size distributions obtained using both techniques were in agreement within analytical uncertainty. The applicability of this technique to the detection of metal-containing protein-binding mesoporous SiO2 microspheres was also investigated. Bound iron (Fe)-containing proteins (i.e., lactoferrin and transferrin) of mesoporous SiO2 microspheres were detected using Fe as a presence marker for the proteins. Thus, spICP-MS is applicable to the particle size measurement of large-sized and non-porous/mesoporous SiO2 microspheres; it has considerable potential for element-based detection and qualification of bound proteins of mes-oporous SiO2 microspheres in a variety of applications.

Remotely sensed data play a crucial role in monitoring the El-Niño/La-Niña Southern Oscillation (ENSO), which is an oceanic-atmospheric phenomenon occurring quasi-periodically with several impacts worldwide, such as specific biological and global climate responses. Since 1980, Earth has witnessed three strong ENSO events (1982–1983, This study aims, 2015-2016). In September 2022, La-Niña entered its third year and was unlikely to continue through 2023. Instead, since 2022 forecasts pointed to a transition from La-Nina to a Neutral phase in summer or late 2023. The onset of the El-Niño occurred around April 2023, and it is anticipated by sophisticated models to be a strong event through the Northern Hemisphere winter (December 2023 - February 2024). The aim of this study is to demonstrate the ability of the combination of two new methods to improve the accuracy of the above claim because El-Niño apart from climate anomalies, significantly impacts Earth’s ecosystems and human societies, regulating the spread of diseases by insects (e.g., malaria and dengue fever), and influencing nutrients, phytoplankton biomass, and primary productivity. This is done by exploring first the previous major El-Niño events in the period January 1876-July 2023. Our calculations show that the ongoing 2023-2024 El-Niño will not be the strongest.

Natural Fibre Composites (NFC) are eco-friendly, sustainable alternatives to synthetic polymers. However, some intrinsic natural fillers´ properties hinder their widespread implementation as re-inforcement in polymeric matrices and require further investigation. In the scope of this study, the thermal, rheologic, mechanical (tension and flexural modes), morphological properties, water ab-sorption and dimensional stability of the NF polypropylene (PP) based injection moulded compo-sites with the rice husk (rh) and olive pits (op) of 20 wt% and 30% wt%, were investigated. The results suggest that the higher content of the rice husk and olive pits led to a similar reduction of the Melt Flow Index (MFI), independent of the additive type compared to virgin polypropylene (PPv). The melting and crystallization temperatures of the PPrh and PPop composites did not change with statistical significance. The composites are stiffer than the PP matrix by up to 49% and possess higher mechanical strength in what concerns the tension mode at the expense of de-creased ductility. PPrh and PPop have superior flexural modulus in the bending mode, while the flexural strength improvement was accomplished for the PP30%rh. The dimensional stability of the composites was improved as the linear shrinkage in the flow direction was decreased by 49% for PPrh and 30% for PPop, positively correlating with an increase in the filler content and stiff-ness. PPop was less susceptible to water sorption than PPrh due to fibres' composition and larger surface-to-area volume ratios.

The article is devoted to the analytical and numerical study of the pattern of propagation and attenuation, due to Coulomb friction, of shear waves in an infinite thin elastic plate with a circular orifice of radius lying on a rough base. In the field of motion, an exact analytical solution of a nonlinear boundary value problem for tangential stresses and transversal velocities is obtained in quadratures by the method of Laplace transformations. It turned out that the complete exhaustion of the wave front of a strong rupture occurs at a finite distance from the center of the hole and an elementary formula is given for this distance (the case of tangential forces instantly applied to the orifice boundary, and then constant in time, is considered). For various ratios of the magnitude of the limiting friction force to the amplitude of the applied load, the trailing wave fronts are obtained, after which a state of static equilibrium between the elastic and friction forces with a nonlinear distribution of residual deformations is established in the region .

Financial time series and other human-driven, non-natural processes are known to exhibit fat-tailed outcome distributions. That is, such processes demonstrate a greater tendency for extreme outcomes than the normal distribution or other natural distributional processes would predict. We examine the mathematical expectation, or simply "expectation," traditionally the probability-weighted outcome, regarded since the seventeenth century as the mathematical definition of "expectation." However, when considering the "expectation" of an individual confronted with a finite sequence of outcomes, particularly existential outcomes (e.g., a trader with a limited time to perform or lose his position in a trading operation), we find this individual "expects" the median terminal outcome over those finite trials, with the classical seventeenth-century definition being the asymptotic limit as trials increase to this. Since such finite-sequence "expectations" often differ in values from the classic one, so do the optimal allocations (e.g., growth-optimal). We examine these for fat-tailed distributions. The focus is on implementation, and the techniques described can be applied to all distributional forms.

Lots of efforts are made to optimize spectrum quality used for indoor farming to maximize artificial light utilization and reduce water loss. For such an improvement, green (G) light supplementation to a red-blue (RB) background was successfully employed in our previous studies to restrict both non-photochemical thermal dissipation (NPQ) and stomatal conductance (gs). At the same time, however, the downregulation of NPQ and gs influences the leaf temperature (Tleaf) in opposite ways. Thus, to elucidate which factor plays a more prominent role in Tleaf regulation and whether such response is temporal or permanent we investigated the correlation between NPQ or gs and the subsequent Tleaf. To this end, we analysed tomato plants (Solanum lycopersicum L. cv. Malinowy Ozarowski) grown solely under monochromatic LED lamps (435, 520, or 662 nm; 80 µmol m–2 s–1) or under mixed RGB spectrum (1:1:1; 180 µmol m–2 s–1), and simultaneously measured the gs and Tleaf with an infrared gas analyser and thermocouple or employed infrared thermal camera (FLIR) during thermal imaging analyses. Results documented that growth light quality significantly modified the Tleaf, and such response is not temporal. Furthermore, we found that plants' actual adaxial leaf surface temperature is more closely related to NPQ amplitude, while the temperature of the abaxial surface corresponds to gs.

5G low-earth orbit satellite communications offer enhanced wireless coverage beyond the reach of 5G terrestrial networks, with important implications, particularly for automated and connected vehicles. Such wireless automotive mass-market applications demand well-designed compact user equipment antenna terminals offering non-terrestrial jointly with terrestrial communications. The antenna should be low-profile, conformal, and meet specific parameter values for gain and operational frequency bandwidth, tailored to the intended applications, in line with the aesthetic design requirements of passenger cars. This work presents an original concept for a dual-band nested circularly polarized automotive user terminal that operates at the S-band frequencies around 3.5 GHz and Ka-band frequencies around 28 GHz, namely within the 5G new-radio bands n78 and n257, respectively. The proposed terminal is designed to be integrated into the plastic components of a passenger vehicle. The arrays consist of 2×2 aperture-coupled corner-truncated microstrip slot patch antenna elements for the n78 band and of 4×4 single-layer edge-truncated microstrip circular slot patch antenna elements for the n257 band. The embedded arrays offer, across the two bands, respectively 9.9 and 13.7 dBi measured realized gain and 3-dB axial ratio bandwidths of 100 and 1500 MHz for the n78 and n257 bands along the broadside direction. Detailed link budget calculations anticipate uplink data rates of 21 and 6 Mbit/s, respectively, deeming it suitable for various automotive mobility and internet-of-things applications.

Diabetes mellitus (DM) and chronic kidney disease (CKD) are common in middle aged and elderly individuals. DM may accelerate the aging process, and the age-related declines in the estimated Glomerular Filtration Rate (eGFR), can pose a challenge to diagnose diabetic kidney disease (DKD) using standard diagnostic criteria especially with absence of severe albuminuria among the elderly. In the presence of CKD and DM, elderly patients may need multidisciplinary care due to susceptibility to various health issues e.g., cognitive decline, auditory or visual impairment, various comorbidities, complex medical regimens, and increased sensitivity to medication adverse effects. As a result, it can be challenging to apply recent therapeutic advancements for the general population to the elderly. We review the evidence that the benefits from these newer therapies apply equally to elderly and non-elderly patients and propose a framework for the management of DKD by discussing the nonpharmacological measures and pharmacological management with renin angiotensin system inhibitors (RASi), Sodium Glucose Co-Transporter two inhibitors (SGLT2i), non-steroidal mineralocorticoids receptor antagonists (MRAs), and Glucagon Like Peptide 1 Receptor Agonists (GLP1-RAs).

Introduction: The objective of the study is to assess the failure of therapies with HFNO (High-Flow Nasal Oxygen), CPAP, Bilevel or combined therapy in patients with hypoxemic acute respiratory failure due to SARS-CoV-2 during their hospitalization. Methods: Retrospective and observational study of SARS-CoV-2 positive patients who required non-invasive respiratory support (NIRS), at the Reina Sofía General University Hospital of Murcia between March 2020 and May 2021. Results: Of 7355 patients, 197 (11,8%) were included. 95 of them failed this therapy (48,3%). We objectivated that during hospitalization in the ward the combined therapy with HFNO and CPAP had a lower failure rate overall and the highest treatment purely with Bilevel (p = 0,005). In the comparison of failure in therapy without two levels of airway pressure, HFNO, CPAP and combined therapy of HFNO with CPAP, (35,6% of patients) presented a 24,2% failure compared to those who did have two levels of pressure with Bilevel and combined therapy of HFNO with Bilevel (64,4% of patients) with 75,8% associated failure (OR: 0, 374; CI 95%: 0,203-0,688. p=0,001).Conclusions: The use of NIRS in conventional hospitalization is safe and effective in patients with respiratory failure secondary to SARS-CoV-2 infection. The therapeutic strategy with Bilevel increases the probability of failure, with the combined therapy strategy with CPAP and HFNO being the most promising option.

Arrhythmic sudden cardiac death (SCD) has an annual prevalence of 1 per 1000 while 75% of the victims suffer from ischemic and 10% from non-ischemic or hypertrophic cardiomyopathy. Altogether, these three entities account for more than 80% of the total SCD victims. Guidelines for implantable cardiac defibrillators are still dominated by LVEF<30% from the MADIT II study. In terms of arrhythmic risk stratification, the PRESERVE-EF study restored in clinical practice the two-step arrhythmic risk stratification approach based on Electrocardiographic non-invasive risk factors (NIRFs) guiding to electrophysiological study. In our times with the multiple cardiac imaging methods and artificial intelligence applications availability, this two-step approach based on integrated arrhythmia mechanisms detection, emerges as an efficient SCD risk stratification paradigm for these three entities but also for the patients with congenital heart disease.

Objective: To evaluate the existing meta-analytic evidence of associations between exposure to ultra-processed foods, as defined by the Nova food classification system, and adverse health outcomes. Design: Systematic umbrella review of existing meta-analyses. Data Sources: MEDLINE, PsycINFO, Embase, and the Cochrane Database of Systematic Reviews, as well as manually searching reference lists from 2009 to June 2023. Inclusion Criteria: Systematic reviews and meta-analyses of cohort, case-control, and/or cross-sectional study designs that evaluated the associations between exposure to ultra-processed foods and adverse health outcomes in humans across the lifespan. Results: The search identified 45 unique pooled analyses, including 13 dose-response and 32 non-dose-response associations (n=9,888,373). Overall, direct associations were found between exposure to ultra-processed foods and 32 (71%) health outcome domains spanning cardiometabolic, gastrointestinal, and respiratory conditions, cancer, mental health, and mortality. Based on a pre-specified evidence classification criteria, convincing evidence (Class I) supported direct associations between greater ultra-processed food exposure and higher risks of incident cardiovascular disease-related mortality (RR: 1.50; 95%CIs: 1.37 to 1.63), type two diabetes (dose-response RR: 1.12; 95%CIs: 1.11 to 1.13), and depressive outcomes (HR: 1.21; 95%CIs: 1.16 to 1.28), as well as higher risks of prevalent anxiety outcomes (OR: 1.48; 95%CIs: 1.37 to 1.59) and combined common mental disorder outcomes (OR: 1.53; 95%CIs: 1.43 to 1.63). Highly suggestive (Class II) evidence indicated that greater exposure to ultra-processed foods was directly associated with higher risks of incident all-cause mortality (RR: 1.21; 95%CIs: 1.15 to 1.27), heart disease-related mortality (dose-response HR: 1.66; 95%CIs: 1.51 to 1.84), and type two diabetes (OR: 1.40; 95%CIs: 1.23 to 1.59), together with higher risks of prevalent obesity (OR: 1.55; 95%CIs: 1.36 to 1.77), adverse sleep-related outcomes (OR: 1.41; 95%CIs: 1.24 to 1.60), and wheezing (RR: 1.40; 95%CIs: 1.27 to 1.55). Out of the remaining 34 pooled analyses, 21 were graded as suggestive or weak strength (Class III-IV) and 13 were graded as no evidence (Class V). Using the GRADE framework, 22 pooled analyses were rated as "Low" quality, with 19 rated as "Very Low” and four rated as "Moderate" quality. Conclusions: Higher exposure to ultra-processed food was associated with a greater risk of adverse health outcomes, especially cardiometabolic, common mental disorder, and mortality outcomes. These findings provide a rationale to evaluate the effectiveness of using population-based measures to target and reduce dietary exposure to ultra-processed foods for improved human health. Due to related uncertainties brought by changes in energy consumption and the integration of rooftop photovoltaic systems, the accuracy and availability of load profiling data are difficult to achieve. However, project managers and planners use a precise load profile as a crucial tool when determining whether the feeder must be updated or de-loaded. The paper aims to create a non-intrusive monitoring system that predicts the synthetic load profiles behavior of energy consumption using a Light Gradient Boosted and Support Vector Machine (SVM) machine learning technique. The most effective ML algorithm is chosen, and it has the potential to be assessed and verified using validation curves, residuals model generation, and prediction error metrics based on the following key statistical indicators: Mean Absolute Error, Mean Square Error, Root Mean Square Error, R-Square, Root Mean Squared Logarithmic Error, and Mean Absolute Percentage Error. The most effective ML has the potential to be assessed and verified using validation curves, residual model generation, and prediction error metrics based on the following key statistical indicators. The result shows that the estimation of the Irms avg -based on LightGBM is more accurate than the SVM because of its quick, economical, and difficult to overfit, particularly with high-dimensional data, speed, and efficiency of the MAE (3.0698), MSE (15.2757), RMSE (3.9020), RMSLE (0.1433), and MAPE (0.0049) respectively. Additionally, the machine learning model shows that, when compared to the SVM, the LightGBM model had the highest accurate prediction, with R-Square values of 89.8%, 90.3%, and 88.5% for Irms_A_avg, Irms_B_avg, and Irms_C_avg, respectively for Brakfontein Substation supply region, and best represents a diverse customer base.

This study aimed to develop extended–release tablets containing 25 mg IMM−H014, an original drug formulated by a direct powder pressing method based on pharmaceutical–grade hydrophilic matrix polymers, such as hydroxypropyl methylcellulose, to establish an in vitro−in vivo corre-lation (IVIVC) to predict bioavailability. The tablets’ mechanical properties and in vitro and in vivo performance were studied. The formulation was optimized using a single factor experiment and the reproducibility was confirmed. The in vitro dissolution profiles of the tablet were de-termined in five dissolution media, in which the drug released from the hydrophilic tablets fol-lowed Ritger–Peppas model kinetics in 0.01 N HCl medium for the first 2 h, and in phosphate buffer saline medium (pH 7.5) for further 24 h. Accelerated stability studies (40°C, 75% relative humidity) proved that the optimal formulation was stable for 6 months. The in vivo pharmaco-kinetics study in beagle dogs showed that compared to the IMM−H014 immediate release prep-aration, the maximum plasma concentration of the extended release (ER) preparation was sig-nificantly decreased, while the maximum time to peak and mean residence time were significantly prolonged. The relative bioavailability was 97.9% based on an area under curve, indicating that the optimal formulation has an obvious ER profile. And a good IVIVC was established, which could be used to predict in vivo pharmacokinetics from the formulation composition.

The rapid advancements in gene therapy have opened up new possibilities for the treatment of genetic disorders, including Duchenne muscular dystrophy, Thalassemia, cystic fibrosis, Hemophilia, and Familial hypercholesterolemia. The utilization of the clustered regularly interspaced short palindromic repeats (CRISPR) - CRISPR-associated protein (Cas) system has revolutionized the field of gene therapy by enabling precise targeting of genes. In recent years, CRISPR/Cas9 has demonstrated remarkable efficacy in treating cancer and genetic diseases. However, the susceptibility of nucleic acid drugs to degradation by nucleic acid endonucleases, necessitates the development of functional vectors capable of protecting the nucleic acids from enzymatic degradation, while ensuring safety and effectiveness. This review aims to explore the biomedical potential of non-viral vector-explore CRISPR/Cas9 systems for the treatment of genetic diseases. Furthermore, it provides a comprehensive overview of recent research advances in viral and non-viral vector-based gene therapy for genetic disorders, including insights from preclinical and clinical studies. Additionally, the review analyzes the current limitations of these delivery systems and proposes avenues for the development of novel nano-delivery platforms.

Spinal Cord Injury in children can be a devastating condition in children with profound implications for their overall health and quality of life. In this review we aim to provide a concise overview of the key aspects associated with SCI in pediatric population. Firstly, we discuss the etiology and epidemiology of SCI in children, highlighting the diverse range of causes. We explore the unique anatomical and physiological characteristics of the developing spinal cord that contribute to the specific challenges faced by pediatric patients. Next, we delve into the clinical presentation and diagnostic methods, emphasizing the importance of prompt and accurate diagnosis to facilitate appropriate interventions. Furthermore, we approach the multidisciplinary management of pediatric SCI, encompassing acute medical care, surgical interventions, and ongoing supportive therapies. Finally, we explore emerging research and innovative therapies in the field and we emphasize the need for continued advancements in understanding and treating SCI in children to improve their functional independence and overall quality of life.

Hepatocellular carcinoma (HCC) is one of the most common cancers and the main cause of cancer-related death globally. Immune dysregulation of CD4+ T cells has been identified as a role in the development of HCC. Nevertheless, the underlying molecular pathways of CD4+ T cells in HCC are not completely known. Thus, a better understanding of the dysregulation of lncRNA-miRNA/mRNA network might yield novel insights into the etiology or progression of HCC. In this study, circulating CD4+ T cells were isolated from the whole blood of 10 healthy controls and 10 HCC patients for next-generation sequencing of the expression of lncRNAs, miRNAs, and mRNAs. Our data showed that there was different expression of 34 transcripts (two lncRNAs, XIST and MIR222HG, 29 mRNA, and 3 other types of RNA) and 13 miRNAs in the circulating CD4+ T cells of HCC patients. The expression of the lncRNA XIST-related miRNAs and their target mRNAs was confirmed using real-time quantitative polymerase chain reaction (qPCR) on samples from 100 healthy controls and 60 HCC patients. The lncRNA–miRNA/mRNA regulation network was created using interaction data generated from ENCORI and revealed there are positive correlations in the infiltration of total CD4+ T cells and negative correlations in the infiltration of Th1 CD4+ T cells.

Analysing and understanding donor behaviour in Non-profit Organisations (NPOs) is challenging due to the lack of human and technical resources. Machine learning (ML) techniques can analyse and understand donor behaviour at a certain level; however, it remains to be seen how to build and design an Artificial Intelligence enabled Decision Support System (AI-enabled DSS) to analyse donor behaviour. Thus, this paper proposes an AI-enabled DSS conceptual design to analyse donor behaviour in NPOs. A conceptual design is created following a Design Science Research approach to evaluate an AI-enabled DSS's initial DPs and features to analyse donor behaviour in NPOs. The evaluation process of the conceptual design applied formative assessment through conducting interviews with stakeholders from NPOs. The interviews were conducted using the Appreciative Inquiry framework to facilitate the process of interviews. The results of analysis based on the interviews provide insightful information not only about the proposed conceptual design for an AI-enabled DSS, but also about what is required to analyse donor behaviour NPOs using DSS. Evaluating the conceptual design results recommend efficiency, effectiveness, flexibility, and useability in the requirements of the AI-enabled DSS. This research contributes to the design knowledge base of AI-enabled DSS for analysing donor behaviour in NPOs. Future research will combine theoretical components to introduce a practical AI-enabled DSS for analysing donor behaviour in NPOs. This research is limited to such analysis on donors who donate money or volunteer time for NPOs.

With the proliferating of video surveillance system deployment and related applications, real-time video analysis is very critical to achieve intelligent monitoring, autonomous driving, etc. It is non-trivial to achieve high accuracy and low latency video stream analysis through the traditional cloud computing. In this paper, we propose a non-orthogonal multiple access (NOMA) based edge real-time video analysis framework with one edge server (ES) and multiple user equipments (UEs). A cost minimization problem composed of delay, energy and accuracy is formulated to improve the QoE of UEs. In order to efficiently solve this problem, we propose the joint video frame resolution scaling, task offloading, and resource allocation algorithm based on the Deep Q-Learning Network (JVFRS-TO-RA-DQN), which effectively overcomes the sparsity of the single-layer reward function and accelerates the training convergence speed. JVFRS-TO-RA-DQN consists of two DQN networks to reduce the curse of dimension, which respectively select the offloading and resource allocation action, the resolution scaling action. Experimental results show that JVFRS-TO-RA-DQN can effectively reduce the cost of transmission and computation, and have better performance in convergence compared to other baseline.

Abstract: This is a retrospective study of patients admitted to Jackson Hospital, Montgomery, Alabama with a diagnosis of COVID-19 from January 1, 2021, to February 15, 2022. The independent variable used in the models were patient sex, age, race, BMI category, daily D-dimer categories, categories of anticoagulation doses (Enoxaparin 40 mg VS. Enoxaparin 80 mg VS Enoxaparin 1mg/kg or equivalent), bleeding episodes, and vaccination status. The three different categories of anticoagulation doses were considered for the purpose of the study. Results: The study reviewed a total of one hundred (100) hospitalized patients. Intermediate-dose anticoagulation was found to be the optimal dose as only fourteen percent (14%) patients died compared to a thirty-six percent (36%) and fifty percent (57%) death rate among those treated with low-dose and high-dose anticoagulation, respectively. The multivariate linear regression model predicting patient oxygen requirements revealed D-Dimer and bleeding status to be statistically significant predictors with a p value of <0.01. For the patients who had a D-dimer value ≥ 2 µg/mL, the oxygenation requirement was predicted to be 31 L higher than those with a D-dimer <2 µg/mL (99% CI; p<0.01). When mean D-dimer and corresponding oxygen requirements were calculated per hospitalization days category, the D-dimer levels and oxygen requirements were noted to follow the same trends indicating both values tended to increase and decrease simultaneously. Conclusion: The study concludes daily D-dimer trends can predict COVID-19 patient survival or daily oxygen requirements indicating D-dimer can be the miracle molecule for COVID-19 prognosis.

The EPR paradox was caused by the provision that quantum variables must have pre-existing values. This type of “hidden property realism” was later falsified by Bell’s Theorem. Accordingly, the physical basis for action-at-a-distance between entangled quanta was removed. Yet, modern interpretations present Bell’s inequality as a Locality Criterion, as if Bell violations can only happen at the quantum level, and only with remote interactions. This is a questionable practice, considering that classical joint measurements also violate such inequalities for mutually exclusive wave properties. In particular, consecutive measurements of polarization produce the same coef-ficients of correlation as parallel measurements with entangled quanta, yet they are explicitly local. Furthermore, it is possible to combine parallel and consecutive measurements of Type I polariza-tion-entangled photons in a single experiment, conclusively showing that quantum Bell violations can be local. Surprisingly, classical phenomena also require nonlocal interpretations if pre-existing properties are taken for granted. Hence, the solution is to reject the models with pre-existing properties for both classical and quantum wave-like phenomena.

Lynch syndrome (LS) is an inherited disorder in which affected individuals have a significantly higher-than-average risk of developing colorectal and non-colorectal cancers, often before the age of 50 years. In LS, mutations in DNA repair genes lead to a dysfunctional post-replication repair system. As a result, the unrepaired errors in coding regions of the genome produce novel proteins, called neoantigens. Neoantigens are recognised by the immune system as foreign and trigger an immune response. Due to the invasive nature of cancer screening tests, universal cancer screening guidelines unique for LS (including colonoscopy) are poorly adhered to by LS variant heterozygotes (LSVH). Currently, it is unclear whether immunogenomic components produced as a result of neoantigen formation can be used as novel biomarkers in LS. We hypothesize that: (i) LSVH produce measurable and dynamic immunogenomic components in blood, and (ii) these quantifiable immunogenomic components correlate with cancer onset and stage. Here, we discuss feasibility and propose the potential to: (a) identify personalised novel immunogenomic biomarkers, (b) reduce invasive cancer screening tests and thus increase non-invasive cancer surveillance, (c) improve compliance and adherence to recommended cancer screening guidelines in LSVH.

This paper reports a new global Mittag-Leffler synchronization criterion with regard to fractional-order hyper-chaotic financial systems by designing the suitable impulsive control and the state feedback controller. The significance of this impulsive synchronization lies in the fact that the backward economic system can synchronize asymptotically with the advanced economic system under the effective impulse macroeconomic management means. Matlab LMI-toolbox is utilized to deduce the feasible solution in numerical example, which shows the effectiveness of the proposed methods. It is worth mentioning that the LMI-based criterion usually requires the activation function of the system to be Lipschitz, but the activation function in this paper is fixed and truly nonlinear, which cannot be assumed to be Lipschitz continuous. This is another mathematical difficulty overcome in this paper.

The following article presents an analysis of the determinants of diabetes using a dataset containing the surveys of 2000 patients from the Frankfurt Hospital in Germany. The data were analyzed using the following models, namely: Tobit, Probit, Logit, Multinomial Logit, OLS, WLS with heteroskedasticity. The results show that the presence of diabetes is positively associated with "Pregnancies", "Glucose", "BMI", "Diabetes Pedigree Function", "Age" and negatively associated with "Blood Pressure". A cluster analysis is realized using the fuzzy c-Means algorithm optimized with the Elbow method and three clusters were found. Finally a confrontation among eight different machine learning algorithms is realized to select the best performing algorithm to predict the probability of patients to develop diabetes.

This paper studies the performance improvement of the IEEE 802.15.4 nonbeacon-enabled mode originated by the inclusion of the Request-To-Send/Clear-To-Send (RTS/CTS) handshake mechanism resulting in frame concatenation. Under IEEE 802.15.4 employing RTS/CTS, the backoff procedure is not repeated for each data frame sent but only for each RTS/CTS set. The maximum throughput and minimum delay performance are mathematically derived for both the Chirp Spread Spectrum and Direct Sequence Spread Spectrum Physical layers for the 2.4 GHz band. Results show that the utilization of RTS/CTS significantly enhances the performance of IEEE 802.15.4 applied to healthcare in terms of bandwidth efficiency.

Degenerative spinal cord (SC) compression is a frequent pathological condition with increasing prevalence throughout aging. Initial non-myelopathic cervical SC compression (NMDC) might progress over time into potentially irreversible degenerative cervical myelopathy (DCM). While quantitative MRI (qMRI) techniques demonstrated the ability to depict intrinsic tissue properties, longitudinal in-vivo biomarkers to identify NMDC patients who will eventually develop DCM are still missing. Thus, we aim to review the ability of qMRI techniques (such as diffusion MRI, diffusion tensor imaging (DTI), magnetization transfer (MT) imaging, magnetic resonance spec-troscopy (1H-MRS)) to serve as prognostic markers in NMDC. While DTI in NMDC patients con-sistently detected lower fractional anisotropy and higher mean diffusivity at compressed levels caused by demyelination and axonal injury, MT and 1H-MRS along with advanced and tract-specific diffusion MRI recently revealed microstructural alterations also rostrally pointing to Wallerian degeneration. Recent studies also disclosed significant relationship between micro-structural damage and functional deficits, assessed by qMRI and electrophysiology, respectively. Thus, tract-specific qMRI in combination with electrophysiology critically extend our under-standing of the underlying pathophysiology of degenerative SC compression and may provide predictive markers of DCM development for accurate patient management. However, the prog-nostic value must be validated in longitudinal studies.

The research and development of non-viral gene therapy has been extensive over the past decade and has received a big push thanks to the successful approval of non-viral gene therapy products in recent times. Despite these developments, gene therapy applications in cancer have been limited. One of the main causes of this has been the imbalance in development of delivery vectors as compared to nucleic acid payloads. This paper reviews non-viral vectors that can be used to deliver nucleic acids for cancer treatment. It discusses various types of vectors and highlights their current applications. Additionally, it also discusses perspective on regulatory landscape to facilitate commercial translation of gene therapy.

Hematological malignancies are considered the fifth most common cancer in the world. Several risk factors and probable etiological agents have been suspected in the pathomechanism of those malignancies as infections, chemicals, irradiation, etc., and recently, the contribution of the altered gut flora, dysbiosis, was identified also as a possible additional factor to the existing ones. Host, and external factors, like antibiotics, which were identified as a major disruptor of the "normal" gut flora, influence the composition of the microbiome. Considering the several-fold differences in antibiotic consumption patterns and the incidence of hematological malignancies in European countries, the hypothesis was raised that the dominant consumption of certain antibiotic classes might influence the incidence of different hematological malignancies through the modification of gut flora. Comparisons were performed between the average antibiotic consumption databases reported yearly by ECDC (2009-2019) and the incidence rate of Hodkin lymphoma (HL), non-Hodgkin lymphoma (NHL), multiple myeloma (MM), and leukemia (LEU) estimated for 2020 in 30 European countries. Applying Spearman calculations, significant positive correlation has been found between the incidence of HL and tetracycline (J01A) consumption (r = 0.399, p = 0,029), NHL and narrow spectrum, beta-lactamase resistant penicillin (J01CF) (r = 0.580, p = 0,001), MM and tetracycline (r = 0.492, p = 0.006), penicillin (J01C) (r = 0.366, p = 0.047), narrow spectrum, beta-lactamase resistant penicillin (J01CF) (r = 0.574, p = 0.001), while strong, significant negative correlation has been recorded between NHL and cephalosporin (r = -0,460, p = 0,011), and quinolone (r = -0,380, p = 0,038). The incidence of LEU did not show any positive or negative association with any antibiotic classes. It is concluded that certain antibiotic classes, in addition to other putative factors, might promote or inhibit the development of different hematological malignancies.

Through a modern derivation of Planck's formula for the entropy of an arbitrary beam of photons we derive a general expression for the entropy production due to the irreversible process of the absorption of an arbitrary incident photon spectrum in material and its dissipation into an infrared-shifted grey-body emitted spectrum, the rest being reflected or transmitted. Employing the framework of Classical Irreversible Thermodynamic theory, we define the generalized thermodynamic flow as the flow of photons from the incident beam into the material and the generalized thermodynamic force is then just the entropy production divided by the photon flow which is the entropy production per unit photon at a given wavelength. We compare the entropy production under sunlight of different inorganic and organic materials (water, desert, leaves and forests) and show that organic materials are the greater entropy producing materials. Intriguingly, plant and phytoplankton pigments (including chlorophyll) have peak absorption exactly where entropy production through photon dissipation is maximal for our solar spectrum $430<\lambda<550$ nm, while photosynthetic efficiency is maximal between 600 and 700 nm. These results suggest that the evolution of pigments, plants and ecosystems has been towards optimizing entropy production rather than photosynthesis. We propose using the wavelength dependence of global entropy production as a biosignature for discovering life on planets of other stars.

The object of this paper is the bending analysis of isotropic rectangular Kirchhoff plates subjected to non-uniform heating (NUH) using the Fourier transform method. The bottom and top surfaces of the plate are assumed to have different changes in temperature, whereas the change in temperature of the mid-surface is zero. According to classical plate theory, the governing equation of the plate contains second derivatives of the NUH; these derivatives are zero by constant value of the NUH, which leads to its absence in the governing equation. This paper presented an approach by which Fourier sine transform was utilized to describe the NUH, while the double trigonometric series of Navier and the simple trigonometric series of Lévy were utilized to describe the deflection curve. Thus, the NUH appeared in the governing equation, which simplified the analysis. Rectangular plates simply supported along all edges were analyzed, bending moments, twisting moments, and deflections being determined. In addition, rectangular plates simply supported along two opposite edges were analyzed; the other edges having various support conditions (free, simply supported, and fixed).

A total of 281 guano samples were collected from caves (N = 181) in 8 European countries (Bulgaria, Czech Republic, France, Hungary, Italy, Romania, Slovakia and Slovenia) and attics in the Czech R. (N = 100). The correlation of detection of mycobacteria between Ziehl-Neelsen (ZN) microscopy and culture examination and qPCR was strong. ZN microscopy was positive in guano from caves (58.6%) more than double than positivity in guano from attics (21.0%; P &lt;0.01). From 89 mycobacterial isolates (73 isolates from cave guano and 16 isolates from attics’ guano) 68 (76.4%) isolates of 19 sp., ssp. and complex were identified as members of 3 Groups (M. fortuitum, M. chelonae, and M. mucogenicum), and 4 Complexes (M. avium, M. terrae, M. vaccae, and M. smegmatis). A total of 20 isolates (22,5%) belonged to risk group 1 (environmental saprophytes), 48 isolates (53.9%) belonged to risk group 2 (potential pathogens), and none of isolates belonged to risk group 3 (obligatory pathogens). When comparing bat guano collected from caves and attics, differences (P&lt;0.01; Mann-Whitney test) were observed for the electrical conductivity, total carbon, total organic and total inorganic carbon. None difference (P&gt; 0.05; Mann-Whitney test) was found for pH and oxidation-reduction potential parameters.

Anemia and thyroid disorders are global health issues that affect all ages but are more apparent in women. In this case, some serological components responsible for IDA and TSH disorders in women have been found actively regulated through a complex steroidal-calcitriol mediated pathway. This research has been investigated the correlation between Calcitriol and the serological components responsible for IDA and TSH disorders in childbearing and non-child-bearing women of different health conditions. Experimental sampling from 452 women suffering from both IDA and TSH disorders were taken, aged between 0 and 70 years. Serological parameters, such as iron, total iron-binding capacity and ferritin, were assessed for IDA profiling, whereas thyroid-stimulating hormone and free thyroxin were for TSH profiling based on the individual’s serum calcitriol status. The resulted serological data were interpreted using sophisticated computer programming language and algorithms for quantitative biochemical analysis. The study resulted in a significant correlation between FT4 and Calcitriol (P<0.0001) for all age groups. TSH also showed strong interactions with the fluctuation of calcitriol level (P<0.0001), except for the children aged below 10 years (P<0.063). The iron, TIBC, TSH, and FT4 showed phenomenal regulation with the steroidal-calcitriol concentration for congenital patients. Unlike the others, ferritin has a substantial connection with Calcitriol (P<0.0064) fluctuation in the serum. To ratify, the concentrations of TSH, FT4, iron, TIBC, and ferritin were found to be significantly interconnected in terms of serum calcitriol level in women suffering from IDA and TSH disorders simultaneously. To understand the accuracy and efficacy of the Calcitriol in IDA and TSH disorders, some other inflammatory markers and parathyroid hormone analysis are need in future studies, besides a large number of samples.

(1) Background: In patients suffering from heart failure, the main causes of death are either he-modynamic failure, or ventricular arrhythmias. The only tool to significantly reduce arrhythmic sudden death is the implantable cardioverter defibrillator (ICD), but not all patients benefit to the same extend of these devices. (2) Methods: The primary outcome of this single center study was defined as cardiovascular death in patients with ischemic and non-ischemic heart failure who have benefited from ICD therapy. The secondary outcomes were death from any cause, sudden cardiac death, ICD-related therapies (appropriate antitachycardia pacing or shock therapy for ventricular tachycardia or fi-brillation) and recurrences of ventricular tachyarrhythmias. (3) Results: A total of 403 consecutive ICD recipients – symptomatic heart failure patients with ICD for the primary prevention of sudden cardiac death – were included retrospectively: 59% is-chemic cardiomyopathy (ICMP) and 41% non-ischemic cardiomyopathy (NICMP). Within a median follow-up period of 36 months, the incidence of cardiovascular mortality was not signif-icantly different in patients with NICMP and ICMP: the primary outcome had occurred in 9 pa-tients (5.4%) in the NICMP group and in 14 patients (5.9%) in the ICMP group (hazard ratio 1; 95%confidence interval [CI] 0.45 to 2.28; p =0.97). All-cause mortality occurred in 14 of 166 pa-tients (8.4%) in NICMP group and 18 of 237 patients (7.6%) in ICMP group. Sudden cardiac death occurred in 2 patients (1.2%) in the NICMP group and in 4 patients (1.7%) in the ICMP group (hazard ratio 0.71; 95% CI, 0.13 to 3.88; P=0.69). The rate of appropriate device therapies was comparable in both groups. (4) Conclusion: In this study, ICD implantation for primary prevention of sudden cardiac death in patients with symptomatic systolic heart failure was associated with similar rates of cardiovas-cular and all-cause mortality in patients with ischemic heart disease, and in patients with heart failure from other causes. NICMP and ICMP showed comparable rates of recurrent ventricular tachyarrhythmias and appropriate ICD therapies.

This article challenges the assumption that journals and peer review are essential for developing, evaluating and disseminating scientific and other academic knowledge. It suggests a more flexible ecosystem, and examines some of the possibilities this might facilitate. The market for academic outputs should be opened up by encouraging the separation of the dissemination service from the evaluation service. Publishing research in subject-specific journals encourages compartmentalising research into rigid categories. The dissemination of knowledge would be better served by an open access, web-based repository system encompassing all disciplines. There would then be a role for organisations to assess the items in this repository to help users find relevant, high-quality work. There could be a variety of such organisations which could enable reviews from peers to be supplemented with evaluation by non-peers from a variety of different perspectives: user reviews, statistical reviews, reviews from the perspective of different disciplines, and so on. This should reduce the inevitably conservative influence of relying on two or three peers, and make the evaluation system more critical, multi-dimensional and responsive to the requirements of different audience groups, changing circumstances, and new ideas. Non-peer review might make it easier to challenge dominant paradigms, and expanding the potential audience beyond a narrow group of peers might encourage the criterion of simplicity to be taken more seriously - which is essential if human knowledge is to continue to progress.

Studies on the characteristics of hazardous air pollutants (HAPs) in the emissions of medium-duty diesel trucks are significantly insufficient compared to that on heavy-duty trucks. This study investigates the characteristics of regulated pollutants and HAPs such as volatile organic compounds (VOCs), aldehydes, and polycyclic aromatic hydrocarbons (PAHs), and estimates non-methane hydrocarbon (NMHC) speciation in the emissions of medium-duty diesel trucks. Ten medium-duty diesel trucks conforming to Euros 5 and 6 were tested for worldwide harmonized light duty driving test cycle (WLTC), new European driving cycle (NEDC), constant volume sampler (CVS)-75, and National Institute of Environmental Research (NIER)-9 using a chassis dynamometer. CO and NMHC emissions were the highest in the NEDC because of its longer low-speed driving time. NOx emissions were the highest in WLTC owing to the influence of thermal NOx in the high-speed phase. Alkanes dominated non-methane volatile compound (NMVOC) emissions owing to the low reaction of the diesel oxidation catalyst. After-treatment system, driving, and engine conditions influenced the individual components of NMVOC emissions. Formaldehyde emissions were the highest among aldehydes irrespective of driving cycles. By sampling the particle-phase of PAHs, we detected benzo(k)fluoranthene and benzo(a)pyrene and estimated the concentrations of the gas-phase PAHs with models to obtain the total PAH concentrations. The toxic equivalency quantities of benzo(k)fluoranthene and benzo(a)pyrene from NIER-9 (cold) for both Euro 5 and Euro 6 vehicles were more than five times higher than that of NIER (hot) and NEDC. In the case of NMHC speciation, formaldehyde emissions were the highest in all the driving cycles. Formaldehyde and benzene must be controlled in the emissions of medium-duty diesel trucks to reduce their health threats. The results of this study will aid in establishing a national emission inventory system for HAPs of mobile sources in Korea.

The epidemiological situation generated by COVID-19 has highlighted the importance of applying non-pharmacological measures. Among these, mass screening of the asymptomatic general population has been established as a priority strategy by carrying out diagnostic tests to limit the spread of the virus. In this article, we aim to evaluate the economic impact of mass COVID-19 screenings of an asymptomatic population through a Cost-Benefit Analysis based on the estimated total costs of mass screening versus health gains and associated health costs avoided. Excluding the value of monetized health, the Benefit-Cost ratio was estimated at approximately 0.45. However, if monetized health is included in the calculation, the ratio is close to 1.20. The monetization of health is the critical element that tips the scales in favour of the desirability of screening. Screenings with the highest return are those that maximize the percentage of positives detected.

Background: Real-world data on the molecular epidemiology of EGFR resistance mutations at or after progression with first- or second-generation EGFR-TKIs in patients with advanced NSCLC are lacking. Methods: This ongoing observational study was carried out by 23 hospital-based physicians in Greece. The decision to perform Cobas® EGFR Mutation Test v2 in tissue and/or plasma at disease progression was made before enrollment. For patients with negative/inconclusive T790M plasma-based results, tissue re-biopsy could be performed. Results: Ninety-six (96) eligible patients were consecutively enrolled (median age: 67.8 years) between July-2017 and September-2019. Of the patients, 98% were tested upon progression using plasma and 2% using tissue/cytology biopsy. The T790M mutation was detected in 16.0% of liquid biopsies. Tissue re-biopsy was performed in 22.8% of patients with a T790M-negative plasma result. In total, the T790M positivity rate was 21.9%, not differing between patients on first- or second-generation EGFR-TKI. Higher (≥2) ECOG performance status and longer (≥10 months) time to disease progression following EGFR-TKI treatment initiation were associated with T790M positivity. Conclusions: Results from plasma/tissue-cytology samples in a real-world setting, yielded a T790M positivity rate lower than previous reports. Fewer than one in four patients with negative plasma-based testing underwent tissue re-biopsy, indicating the challenges in routine care settings.

Diffuse large B cell lymphoma (DLBCL) is curable with chemoimmunotherapy in ~65% of patients. One of the hallmarks of the pathogenesis and resistance to therapy in DLBCL is inhibition of apoptosis, which allows malignant cells to survive and acquire further alterations. Inhibition of apoptosis can be the result of genetic events inhibiting the intrinsic or extrinsic apoptotic pathways, as well as their modulators, such as the inhibitor of apoptosis proteins, P53, and components of the NF-kB pathway. Mechanisms of dysregulation include upregulation of anti-apoptotic proteins and downregulation of pro-apoptotic proteins via point mutations, amplifications, deletions, translocations, and influences of other proteins. Understanding the factors contributing to resistance to apoptosis in DLBCL is crucial in order to be able to develop targeted therapies that could improve outcomes by restoring apoptosis in malignant cells. This review describes the genetic events inhibiting apoptosis in DLBCL, provides a perspective of their interactions in lymphomagenesis, and discuss their implication for the future of DLBCL therapy.

The evolution of integrable classical systems leads to conserved quantities and vanishing Poisson brackets. In contrast, such invariants do not exist in the dynamics of non-integrable systems, which include (but are not limited to) deterministic models with long-term chaotic behavior. The object of this review is to briefly survey the mathematical background of nonintegrability and its role in the physics unfolding well-above the Standard Model (SM) scale.

Abstract: The objective of this meta-analysis was to evaluate the factors associated with mortality of elderly Italians diagnosed with the new coronavirus who resided in institutions or who were hospitalized as a result of the disease. Methods: A systematic review following the recommenda-tions of The Joanna Briggs Institute (JBI), where the PEO strategy was utilized - Population, Exposure and Outcome. P, being the elderly over 65 years old. E, the SARS-CoV_2 pandemic. O, mortality. The NCBI / PubMed, LILACS, EMBASE and CINAHL databases were used until July 31, 2020.; Results: Five Italian studies were included in the meta-analysis, with the number of elderly people varying between 18 and 1591 patients. The main morbidities presented by the elderly in the studies were: dementia, diabetes, chronic kidney disease and hypertension. Conclusions: The factors as-sociated with the mortality of elderly Italian people diagnosed with SARS-CoV-2 who lived in in-stitutions or who were hospitalized because of the disease were evaluated. It was found that de-mentia, diabetes, chronic kidney disease and hypertension are the main the main diagnosed dis-eases for mortality in elderly people with Covid-19.

There will be a surge in demand and investment for hydrogen economy post Covid-19.

Arthropod-borne viruses belonging to the flavivirus genus possess an enormous relevance in public health. Neotropical non-human primates (NPs) have been proposed to be infected more frequently with flaviviruses due to their arboreal and diurnal habits, their genetic similarity to humans and their relative closeness to humans. However, the only known flavivirus in America that is maintained by sylvatic cycles involving NPs is Yellow Fever virus (YFV), and the NPs role as potential hosts of flaviviruses is still unknown. Here, we examined flavivirus exposure in 86 free range and captive NPs of Costa Rica to evaluate their involvement in flavivirus transmission cycles and their potential as flavivirus hosts. We used a highly-specific micro plaque reduction neutralization test (micro-PRNT) to determine the presence of antibodies against YFV, Dengue virus 1-4 (DENV), Zika virus, West Nile Virus (WNV) and Saint Louis Encephalitis virus (SLEV). We found evidence of seropositive NPs to DENV-1 8.2% (homotypic –3/86, heterotypic – 4/86), SLEV 15.1% (homotypic – 10/86, heterotypic – 2/86), WNV 2.3% (homotypic – 2/86) and 8.1% (7/86) undetermined Flavivirus species. No antibodies against YFV or ZIKV were found. This work provides compelling serological evidence of exposure in NPs of flaviviruses associated with urban cycles, i.e. DENV, and confirms decades of circulation of SLEV in the same environments. Also, the range of years of sampling and the socioeconomic region was statistically significant for the presence of Dengue and Flavivirus undetermined seropositive individuals, respectively. Both the years and socioeconomic regions with greater seroprevalence coincide with the years and socioeconomic regions with high numbers of Dengue human cases for the country. Our work suggests bidirectional? circulation of different flaviviruses between humans and wildlife with public health importance and underscores the necessity of further surveillance for flaviviruses in the humans/wildlife interface in Central America.

Lift-offs of the sensor could significantly affect the measurement signal and reconstruction of material properties when using the electromagnetic (inductive) eddy current sensor. Previously, various methods (including novel sensor designs, and features like zero-crossing frequency, lift-off point of intercept) have been used for eliminating the measurement error caused by the lift-off distance effect of the sensor. However, these approaches can only be applied for a small range of lift-off variations. In this paper, a linear relationship has been found between the sensor lift-off and ratio of dual-frequency eddy current signals, particularly under the high working dual frequencies. Based on this linear relationship, the lift-off variation can be reconstructed firstly with a small error of 2.5 % when its actual value is up to 10 mm (10.1 % for 20 mm). The reconstructed lift-off is used to further get the property of the material under a low single frequency. Experiments on different ferrous metals have been carried out for the testing of the reconstruction scheme. Since the inductance is more sensitive to the material property (and less sensitive to the lift-off) under low frequencies, the reconstruction error of the material property is much smaller than that of the lift-off, with 1.4 % under 12 mm (and 4.5 % under 20 mm).

The main purpose of this paper is to propose a novel optimization model with a new machine learning approach in the first section to achieve the best results in financial institutions in the second section. Since the constancy of efficacy derived from parametric and non-parametric is not significant, this paper provides a scientific assessment at the optimization section and proposes a novel combined parametric and non-parametric model which will be a new experiment in literature perception. A scientific assessment of banks based on a combination of the efficiency measurement method of CCR(Charnes, Cooper and Rhodes model) or CRS(Constant Return to Scale) BCC(Banker, Charnes, and Cooper model) or VRS (Variable Return to Scale) in Data Envelopment Analysis (DEA), as well as Stochastic Frontier Approach (SFA) for 65 banks during Feb to July 2020, are introduced. For analyzing the performance of the parametric and non-parametric approaches we have considered the linear regression and Unreplicated Linear Functional Relationship (ULFR). At the machine learning section, a novel four-layers data mining filtering pre-processes for selected supervised classification as well as unsupervised clustering algorithms to increase the accuracy and to remove unrelated attributes and data are applied. For the four kinds of preprocessing approaches of unsupervised attributes, supervised attributes, supervised instances, and unsupervised instances, we have chosen discretization, attribute selection, stratified remove folds, and resample filters respectively. Based on the nature of the suggested financial institution's dataset and attributes, the most appropriate preprocessing filter in each layer to achieve the highest performance is suggested. Finally, the superior bank, best performance model, and the most accurate algorithm are introduced. The results indicate that the bank number 56 is the superior bank. Among the proposed techniques, the novel recommended CVS compared with CCR-BCC and SFA models, has a more positive correlation with profit risk, and show a higher coefficient of determination values. Sequential Minimal Optimization(SMO) algorithm receives the highest accuracy in all four suggested filtering layers.

Asians are known to more likely than Westerners develop fatty liver and lifestyle-related diseases despite their weight. However, the relationship between fat accumulation and lifestyle-related diseases in non-obese Asians is unknown. Therefore, this study aimed to analyze visceral fat and hepatic fat in participants with a normal body mass index (BMI) and examine their characteristics during a medical checkup. This cross-sectional study was conducted on 663 of 1,142 patients who underwent abdominal ultrasonography and who had an alcohol intake (converted to ethanol) of <30 g/day for males and <20 g/day for females and a BMI of <25 kg/m2 during a health checkup. Participants were classified into four groups: group A, visceral fat accumulation (VFA) (−) and fatty liver (FL) (−) (n = 549); group B, VFA (+) and FL(−) (n = 32); group C, VFA (−) and FL (+) (n = 58); and group D, VFA (+) and FL (+) (n = 24). The frequencies of lifestyle-related disease complications, liver function tests, and liver fibrosis were evaluated among the four groups. Compared with group A (control), groups B, C, and D had higher number of males; BMI; abdominal circumference, ALT, AST, γ-GTP, triglyceride, uric acid, fasting blood sugar levels; and incidence of hyperlipidemia. Groups C and D had higher ALT, HbA1c, cholinesterase, and triglyceride levels; FIB4 index; and number of patients with diabetes mellitus (DM) than groups A and B; however, there was no difference between groups A and B. FL is a risk factor of DM and liver fibrosis in non-obese Japanese individuals; however, VFA only is not a risk factor of DM and liver fibrosis.

Previously, a conductivity invariance phenomena (CIP) has been discovered – at a certain lift-off, the inductance change of the sensor due to a test sample is immune to conductivity variations, i.e. the inductance – lift-off curve passes through a common point at a certain lift-off, termed as conductivity invariance lift-off. However, this conductivity invariance lift-off is fixed for a particular sensor setup, which is not convenient for various sample conditions. In this paper, we propose using two parameters in the coil design – the horizontal and vertical distances between the transmitter and the receiver to control the conductivity invariance lift-off. The relationship between these two parameters and the conductivity invariance lift-off is investigated by simulation and experiments and it has been found that there is an approximate linear relationship between these two parameters and the conductivity invariance lift-off. This is useful for applications where the measurements have restrictions on lift-off, e.g. uneven coating thickness which limits the range of the lift-off of probe during the measurements. Therefore, based on this relationship, it can be easier to adjust the configuration of the probe for a better inspection of the test samples.

Neural tissue engineering, nanotechnology and neuroregeneration are diverse biomedical disciplines that have been working together in recent decades to solve the complex problems linked to central nervous system (CNS) repair. It is known that the CNS demonstrates a very limited regenerative capacity because of a microenvironment that impedes effective regenerative processes, making development of CNS therapeutics challenging. Given the high prevalence of CNS conditions such as stroke that damage the brain and place a severe burden on afflicted individuals and on society, it is of utmost significance to explore the optimum methodologies for finding treatments that could be applied to humans for restoration of function to pre-injury levels. Extracellular vesicles (EVs), also known as exosomes, when derived from mesenchymal stem cells, are one of the most promising approaches that have been attempted thus far, as EVs deliver factors that stimulate recovery by acting at the nanoscale level on intercellular communication while avoiding the risks linked to stem cell transplantation. At the same time, advances in tissue engineering and regenerative medicine have offered the potential of using hydrogels as bio-scaffolds in order to provide the stroma required for neural repair to occur, as well as the release of biomolecules facilitating or inducing the reparative processes. This review introduces a novel experimental hypothesis regarding the benefits that could be offered if EVs were to be combined with biocompatible injectable hydrogels. The rationale behind this hypothesis is presented, analyzing how a hydrogel might prolong the retention of EVs and maximize the localized benefit to the brain. This sustained delivery of EVs would be coupled with essential guidance cues and structural support from the hydrogel until neural tissue remodeling and regeneration occur. Finally, the importance of including non-human primate (NHP) models in the clinical translation pipeline, as well as the added benefit of multi-modal neuroimage analysis to establish non-invasive, in vivo, quantifiable imaging-based biomarkers for CNS repair are discussed, aiming for more effective and safe clinical translation of such regenerative therapies to humans.

Previously, we demonstrated that IR triggers the invasion/migration of A549 cells via activation of an EGFR–p38/ERK–STAT3/CREB-1–EMT pathway. Here, we have demonstrated the involvement of a novel intracellular signaling mechanism in γ-ionizing radiation (IR)-induced migration/invasion. Expression of receptor-interacting protein (RIP) 1 was initially increased upon exposure of A549, a non-small cell lung cancer (NSCLC) cell line, to IR. IR-induced RIP1 is located downstream of EGFR and involved in the expression/activity of matrix metalloproteases (MMP-2 and MMP-9) and vimentin, suggesting a role in epithelial-mesenchymal transition (EMT). Our experiments showed that IR-induced RIP1 sequentially induces Src-STAT3-EMT to promote invasion/migration. Inhibition of RIP1 kinase activity and expression blocked induction of EMT by IR and suppressed the levels and activities of MMP-2, MMP-9, and vimentin. IR-induced RIP1 activation was additionally associated with stimulation of the transcriptional factor NF-κB. Specifically, exposure to IR triggered NF-κB activation and inhibition of NF-κB suppressed IR-induced RIP1 expression followed by a decrease in invasion/migration as well as EMT. Based on the collective results, we propose that IR concomitantly activates EGFR and NF-κB and subsequently triggers the RIP1–Src/STAT3–EMT pathway, ultimately promoting metastasis.

Biological invasions have reached an unprecedented level and the number of introduced species is still increasing worldwide. Despite major advances in invasion science, the determinants of success of introduced species, the magnitude and dimensions of their impact, and the mechanisms sustaining successful invasions are still debated. Empirical studies show divergent impacts of non-native populations on ecosystems and contrasting effects of biotic and abiotic factors on the dynamics of non-native populations; this is hindering the emergence of a unified theory of biological invasions. We propose a synthesis that merges perspectives from population, community, and ecosystem levels. Along a timeline of ecosystem transformation driven by non-native species, from historical to human-modified ecosystems, we order invasion concepts and theories to clarify their chaining and relevance during each step of the invasion process. This temporal sorting of invasion concepts shows that each concept is relevant at a specific stage of the invasion. Concepts and empirical findings on non-native species may appear contradictory. However, we suggest that, when mapped onto an invasion timeline, they may be combined in a complementary way. An overall scheme is proposed to summarise the theoretical dynamics of ecosystems subjected to invasions. For any given case study, this framework provides a guide through the maze of theories and should help choose the appropriate concepts according to the stage of invasion.

The classical approach to non-linear regression in physics, is to take a mathematical model describing the functional dependence of the dependent variable from a set of independent variables, and then, using non-linear fitting algorithms, extract the parameters used in the modeling. Particularly challenging are real systems, characterised by several additional influencing factors related to specific components, like electronics or optical parts. In such cases, to make the model reproduce the data, empirically determined terms are built-in the models to compensate for the impossibility of modeling things that are, by construction, impossible to model. A new approach to solve this issue is to use neural networks, particularly feed-forward architectures with a sufficient number of hidden layers and an appropriate number of output neurons, each responsible for predicting the desired variables. Unfortunately, feed-forward neural networks (FFNNs) usually perform less efficiently when applied to multi-dimensional regression problems, that is when they are required to predict simultaneously multiple variables that depend from the input dataset in fundamentally different ways. To address this problem, we propose multi-task learning (MTL) architectures. These are characterized by multiple branches of task-specific layers, which have as input the output of a common set of layers. To demonstrate the power of this approach for multi-dimensional regression, the method is applied to luminescence sensing. Here the MTL architecture allows predicting multiple parameters, the oxygen concentration and the temperature, from a single set of measurements.

An increasing number of aircraft is equipped with wing tip devices, which either are installed by the aircraft manufacturer at the production line or are retrofitted after the delivery of the aircraft to its operator. Installation of wing tip devices has not been a popular choice for regional turboprop aircraft and the novelty of the current study is to investigate the feasibility of retrofitting the British Aerospace (BAe) Jetstream 31 with an appropriate wing tip device (or winglet) to increase its cruise range performance, taking also into account the aerodynamic and structural impact of the implementation. To the best of the knowledge of the authors, no previous study exists which has attempted to assess the winglet retrofit of an existing aircraft type of similar size and operating profile. The optimal winglet design achieved a 2.38% increase of the maximum range by reducing the total drag by 1.19% at a mass penalty of 3.25%, as compared with the baseline aircraft configuration. Other designs were found to be more effective in reducing the total drag, but the structural reinforcement required for their implementation outweighed the achieved performance improvements. Since successful winglet retrofit programs for typical short to medium range narrow body aircraft report even more than 3% of block fuel improvements, undertaking the project of installing an optimal winglet design to the BAe Jetstream 31, should also consider a Direct Operating Cost (DOC) assessment on top of the aerodynamic and structural aspects of the retrofit.

In this study, the multi-relaxation-time lattice Boltzmann method is applied to investigate the oscillatory instability of lid-driven flows in two-dimensional semi-elliptical cavities with different vertical to horizontal aspect ratios K in the range of 1.0--3.0. The program implemented in this study is parallelized using CUDA (compute unified device architecture), a parallel computing platform, and computations are carried out on NVIDIA Tesla K40c GPU. To carry out precise calculations, the CUDA algorithm is extensively investigated, and its parallel efficiency indicates that the maximum speedup is 47.6 times faster. Furthermore, the steady--oscillatory Reynolds numbers are predicted by implementing the CUDA-based programs. The amplitude coefficient is defined to quantify the time-dependent oscillation of the velocity magnitude at the monitoring point. The simulation results indicate that the transition Reynolds numbers correlate negatively with the aspect ratio of the semi-elliptical cavity, and are smaller than those of the rectangular cavity at the same aspect ratio. In addition, the detailed vortex structures of the semi-elliptical cavity within a single period are also investigated when the Reynolds number is larger than the steady--oscillatory value to determine the effects of periodic oscillation of the velocity magnitude.

Easter Island (Rapa Nui) deforestation has traditionally been viewed as a single event, synchronous in time and space across the island and caused by Polynesian settlers. However, recent studies have challenged this idea introducing the concept of spatio-temporal heterogeneity and suggesting a role for climate change. This paper presents a continuous paleovegetation record of the last millennium (~960 to ~1710 CE), based on palynological analysis of a peat core from Lake Kao. During this time interval, deforestation was gradual, with three main pulses at ~1070 CE, ~1410 CE and ~1600 CE, likely driven by drought, anthropogenic practices (mostly fire) or the coupling of both. Some forest regeneration trends have been documented after the first and the second deforestation pulses. Forests were totally removed by 1600 CE, coinciding with the full permanent human settlement of the Kao area. Comparison with other continuous palynological records available for the last millennium (Aroi marsh and Lake Raraku), confirms that forest clearing was heterogeneous in time and space, rather than synchronous island-wide.

Causal relationships lie at the very core of our scientific description of biophysical phenomena. Nevertheless, observable facts such as chaotic dynamics, symmetry breaks, long-range collisionless neural interactions, zero-value energy singularities, particle/wave duality, ecological approaches to visual perception elude cause effect inductive explanations. We illustrate how simple topological claims, seemingly useless and far away from scientific inquiry (e.g., “given at least some wind on Earth, there must at all times be a cyclone or anticyclone somewhere”; “if one stirs to dissolve a lump of sugar in a cup of coffee, it appears there is always a point without motion”; “at any moment, there is always a pair of antipodal points on the Earth’s surface with equal temperatures and barometric pressures”) describe a real world, made of projections and mappings unsupplied with causality. Unavoidable, necessary, passive, spontaneous changes in systems’ shapes, dimensions and symmetries incarnate biophysical quantifiable counterparts of abstract mathematical concepts. Real features arising from topological mechanisms do not require causal powers to be produced, because are independent of exerted physical forces, energetic fields’ actions and thermodynamic constraints. Providing examples that shed novel interpretative light on phenomena such as double slit experiments, cellular mechanisms and brain function, we conclude as follows: countless scientific matters can be accessed through methodological weapons which not just break causality, replace temporal correlations and probabilistic a priori knowledge of previous cases, but also permit augments in available information devoid of entropy increases and energy losses.

This work investigates the use of Non-destructive tests as a tool for monitoring the structural performance of concrete structures. The investigation encompassed four phases; the first of which involved the use of destructive and non-destructive mechanisms to assess concrete strength on cube specimens. The second phase research focused on site assessment for a twin engineering theatre located at the Faculty of Engineering, University of Lagos using rebound hammer and ultrasonic pulse velocity tester. The third phase was the use of linear regression analysis model with MATLAB to establish a relationship between calibrated strength as well as ultrasonic pulse velocities with their corresponding compressive strength values on cubes and values obtained from existing structures. Results show that the root-mean squared-R² values for rebound hammer ranged between 0.275 and 0.742 while ultrasonic pulse velocity R² values were in the range of 0.649 and 0.952 for air curing and water curing systems respectively. It initially appeared that the Ultrasonic pulse velocity was more suitable for predicting concrete strength than rebound hammer but further investigations showed that the latter was adequate for early age concrete while the former was more suited for aging concrete. Hence, a combined use is recommended in this work.

Neuroscience has overwhelmingly and understandably focused on the synaptic modality of signal transmission. There is evidence, however, that from an evolutionary perspective, non-synaptic transmission (NST) preceded synaptic signaling. Moreover, in modern nervous systems, NST coexists and extensively interacts with synaptic transmission modifying neuronal dynamics. In fact, NST remains widespread in complex animals, especially within the interoceptive system where the dearth of insulating barriers such as myelin sheaths and the blood-brain barrier enhances the communication between neural and non-neural tissues mediated by NST. We suggest that this physiological arrangement makes a fundamental contribution to interoception¾the process of sensing visceral states¾which is an essential underpinning of the capacity to feel and the foundation of affective processing.

Anticancer regimens for Hodgkin lymphoma (HL) patients include highly genotoxic drugs that have been very successful in killing tumor cells and providing a 90% disease-free survival at five years. However, these treatments do not have a specific cell target, damaging both cancerous and normal cells. Thus, HL survivors have a high risk of developing new primary cancers, both hematologic and solid tumors, that have been related to treatment. Several studies have shown that after-treatment, HL patients and survivors present persistent chromosomal instability, including non-clonal chromosomal aberrations. The frequency and type of chromosomal abnormalities appear to depend on the type of therapy and the cell type examined. For example, MOPP chemotherapy affects hematopoietic and germ stem cells leading to long-term genotoxic effects and azoospermia, while ABVD chemotherapy affects transiently sperm cells, with most of the patients showing recovery of spermatogenesis. Both regimens have long-term effects in somatic cells, presenting non-clonal chromosomal aberrations and genomic chaos in a fraction of non-cancerous cells. This is a source of karyotypic heterogeneity that could eventually generate a more stable population acquiring clonal chromosomal aberrations and leading towards the development of a new cancer.

This study aimed to characterize the long non-coding RNA (lncRNA) expression in the bovine mammary gland and to infer their functions in dietary response to 5% linseed oil (LSO) or 5% safflower oil (SFO). Twelve cows (six per treatment) in mid lactation were fed a control diet for 28 days followed by a treatment period (control diet supplemented with 5% LSO or 5% SFO) of 28 days. Mammary gland biopsies were collected from each animal on day-14 (D-14, control period), D+7 (early treatment period) and D+28 (late treatment period) and were subjected to RNA-Sequencing and subsequent bioinformatics analyses. Functional enrichment of lncRNA was performed via potential cis regulated target genes located within 50 Kb flanking regions of lncRNAs and having expression correlation of >0.7 with mRNAs. A total of 4955 lncRNAs (325 known and 4630 novel) were identified which potentially cis targeted 59 and 494 genes in LSO and SFO treatments, respectively. Enrichments of cis target genes of lncRNAs indicated potential roles of lncRNAs in immune function, nucleic acid metabolism and cell membrane organization processes as well as involvement in Notch, cAMP and TGF-β signaling pathways. Thirty-two and 21 lncRNAs were differentially expressed (DE) in LSO and SFO treatments, respectively. Six genes (KCNF1, STARD13, BCL6, NXPE2, HHIPL2 and MMD) were identified as potential cis target genes of six DE lncRNAs. In conclusion, this study indicated potential roles for lncRNAs in mammary gland immune functions and development and provided potential candidate genes and pathways via which lncRNAs can function in diet responses.

This paper seeks to explain the rapid growth of the Islamic State of Iraq & Syria (ISIS) and approach the question of “what is” the Islamic State? The paper offers several contributions. First is the proposal of a dynamic hypothesis that ISIS is an emerging-state actor and differs notably from traditional non-state actors and insurgencies. The theory consists of both a causal loop diagram and key propositions. A detailed system dynamics simulation (E-SAM) was constructed to test the theory. The propositions of emerging-state actor theory are constructed as synthetic experiments within the simulation and confirm evidence of emerging-state actor behavior. E-SAM’s novelty is its combination of combat simulation with endogenous geospatial feedback, ethnographic behavior in choosing sides in conflict, and details internal simulation of key actor mechanisms such as financing, recruiting and governance. E-SAM can be loaded with scenarios to simulate non-state actors in different geospatial domains: ISIS in Libya, Boko Haram in Nigeria, Taliban in Afghanistan and even expatriated ISIS fighters returning to pursue new conflicts such as in Indonesia.

Telemedicine is the use of Information and Communication Technology (ICT) for clinical health care from a distance. The exchange of radiographic images and electronic patient health information/records (ePHI/R) for diagnostic purposes has the risk of confidentiality, ownership identity, and authenticity. In this paper, a data hiding technique for ePHI/R is proposed. The color information in the cover image is used for key generation, and stego-images are produced with ideal case. As a result, the whole stego-system is perfectly secure. This method includes the features of watermarking and steganography techniques. The method is applied to radiographic images. For the radiographic images, this method resembles watermarking, which is an ePHI/R data system. Experiments show promising results for the application of this method to radiographic images in ePHI/R for both transmission and storage purpose.

Suitable measures of grazing impacts on ground cover, that enable separation of the effects of climatic variations, are needed to inform land managers and policy makers across the arid rangelands of the Northern Territory of Australia. This work developed and tested a time-series, change-point detection method for application to time series of vegetation fractional cover derived from Landsat data to identify irregular and episodic ground-cover growth cycles. These cycles were classified to distinguish grazing impacts from that of climate variability. A measure of grazing impact was developed using a multivariate technique to quantify the rate and degree of ground cover change. The method was successful in detecting both long term (> 3 years) and short term (< 3 years) growth cycles. Growth cycle detection was assessed against rainfall surplus measures indicating a relationship with high rainfall periods. Ground cover change associated with grazing impacts was also assessed against field measurements of ground cover indicating a relationship between both field and remotely sensed ground cover. Cause and effects between grazing practices and ground cover resilience can now be explored in isolation to climatic drivers. This is important to the long term balance between ground cover utilisation and overall landscape function and resilience.

This study sought to investigate steel frames’ performance with dual lateral loader system (Frame bending + bracings of divergent and convergent) in near and far filed to fault. In order to this, four categories of steel frame with dual system with 8, 10 and 12 story are designed with average formation based on existing seismic regulation in 2800 standard of Iran and tenth chapter of national regulations of construction (planning and performing steel construction). Time history non-linear dynamic analysis under the effect of near and far field earthquakes has been done on plan’s models. Then the maximum of floors’ dislocation, floors’ drift, roof dislocation, base shear and energy curves of frames are shown and compared with each other. All non-linear time history analyses have been accomplished using PERFORM 3D software.

Radio loud active galactic nuclei are composed of different spatial features, each one characterized by different spectral properties in the radio band. Among them, blazars are the most common class of sources detected at gamma-rays by \fermi, and their radio emission is dominated by the flat spectrum compact core. In this contribution, we explore the connection between emission at high energy revealed by \fermi\ and at radio frequency. Taking as a reference the strong and very highly significant correlation found between gamma rays and cm-$\lambda$ radio emission, we explore different behaviours found as we change the energy range in gamma rays and in radio, therefore changing the physical parameters of the zones involved in the radiation emission. We find that the correlation weakens when we consider (1) gamma rays of energy above 10 GeV (except for high synchrotron peaked blazars) or (2) low frequency radio data taken by the Murchison Widefield Array; on the other hand, the correlation strengthens when we consider mm-$\lambda$ data taken by ALMA.

QSO B0218+357 is a blazar located at a cosmological redshift of 0.944. The source is gravitationally lensed by a spiral galaxy located at the redshift of 0.68. Strong gravitational lensing splits the signals emitted by the source into two components separated by 10-12 days, as observed in radio and gamma-ray bands. In July 2014 a GeV flare was observed by Fermi-LAT, triggering follow-up observations with the MAGIC telescopes at energies above 100 GeV. The expected time delay between the components allowed us to plan broad band MWL observations before, during and after the trailing component of the emission. The MAGIC observations at the expected time of arrival of the trailing component resulted in the first detection of QSO B0218+357 in Very-High-Energy (VHE, >100 GeV) gamma rays. It is both the farthest known VHE object and one of only a few Flat Spectrum Radio Quasars detected in this energy range. We report here the observed multiwavelength spectral and temporal properties of the emission during the 2014 flare.

Non-Destructive Testing (NDT) is an appealing technique for confirming the welding quality and ensuring structural integrity without causing damage. It assists in regulatory compliance, risk mitigation, and process optimization for improved industrial reliability. This study reviews potential NDT approaches. Through a rigorous analysis of existing surveys, we aim to decipher the current landscape and highlight the significant advancements in the field. Because of the potential of artificial intelligence (AI) assistive X-ray imaging-based NDT, we particularly examine the integration of AI algorithms and X-ray imaging in the NDT of welds. This convergence represents a paradigm shift, redefining traditional methods and ushering in a new era of precision, automation, and efficiency. As we navigate through potential X-ray imaging datasets, delve into crucial image processing techniques, examine feature extraction methods, and explore AI algorithms, our survey reveals the intricate interplay of technologies that drive automated weld defect categorization. The review broadens its focus by including various practical applications, highlighting the adaptable utility of AI-assistive X-ray imaging for weld defect detection in potential industrial applications. Moreover, the promising opportunities and nuanced challenges associated with integrating X-ray imaging and AI in weld integrity assessment are highlighted. It provides a comprehensive perspective on this rapidly evolving field.

Globally, the transport sector is a major contributor to air pollution. Currently in the UK, vehicle emissions contribute significant amounts of nitrogen oxide (NOx) and particulate matter (PM) pollution in urban areas. Low-emission zones policies have been used as an intervention to tackle air pollution, and in this context, the UK launched the Low- Traffic Neighbourhood scheme. This study investigates the impacts of the Low- Traffic Neighbourhood in Kings Heath, Birmingham, UK, to evaluate its impact in reducing air pollution and local community perspectives about the scheme and perceived impacts on health and well-being. The study employs a mixed-method ap-proach comprising air quality monitoring assessment and a survey questionnaire involving 210 residents. The findings reveal an increase in active travel and a reduction in air pollution levels in the years after the implementation of the scheme, although the area is still non-compliant with the 2021 WHO air quality guidelines. Nonetheless, the scheme has a polarizing effect and created a division within the local community about the overall scheme acceptance and spatial distribution of the scheme’s benefits. The study underscores the importance of comprehensive baseline data, long-term community engagement, and integration with broader urban planning initiatives to enhance the success of future Low-Neighbourhood Traffic schemes.

In the field of Thermal Non-Destructive Testing and Evaluation (TNDT&E), active thermography gained popularity due to its fast, wide area monitoring and remote inspection capability to assess materials without compromising their future usability. Among the various active thermographic methods, pulse compression favourable frequency-modulated thermal wave imaging stands out for its enhanced detectability and depth resolution. In this study, experimental investigation has been carried out on a hardened steel sample used in ship building industry with a flat-bottom hole simulated defect using the Frequency Modulated Thermal Wave Imaging (FMTWI) technique. And the defect detection capabilities of FMTWI have been investigated from various statistical post processing approaches and compared by taking Signal to Noise Ration (SNR) as a figure of merit. Among various adopted statistical post-processing techniques, the pulse compression has been carried out using different methods: offset removal with polynomial curve fitting, and Principal Component Analysis (PCA) which is an unsupervised learning approach for data reduction and offset removal with median centering for data standardization. The performance of these techniques was assessed through experimental investigations on hardened steel specimen used in ship building to provide valuable insights into their effectiveness in defect detection capabilities.

The modulation of vocal elements such as pitch, loudness, and duration plays a crucial role in conveying both linguistic information and the speaker’s emotional state. While acoustic features like fundamental frequency (F0) variability have been widely studied in emotional speech analysis, challenges remain in accurately classifying emotions due to the complex and dynamic nature of vocal expressions. Traditional analytical methods often oversimplify these dynamics, potentially overlooking intricate patterns indicative of specific emotions. This study aims to enhance emotion classification in speech by directly incorporating dynamic F0 contours into the analytical framework using Generalized Additive Mixed Models (GAMMs). We utilized the Ryerson Audio-Visual Database of Emotional Speech and Song (RAVDESS), focusing on eight distinct emotional states expressed by 24 professional actors. Sonorant segments were extracted, and F0 measurements were converted into semitones relative to a 100 Hz baseline to standardize pitch variations. By employing GAMMs, we modeled non-linear trajectories of F0 contours over time, accounting for both fixed effects (emotions) and random effects (individual speaker variability). Our analysis revealed that incorporating emotion-specific non-linear time effects and individual speaker differences significantly improved the model’s explanatory power, ultimately explaining up to 66.5% of the variance in F0. The inclusion of random smooths for time within speakers captured individual temporal modulation patterns, providing a more accurate representation of emotional speech dynamics. The results demonstrate that dynamic modeling of F0 contours using GAMMs enhances the accuracy of emotion classification in speech. This approach captures the nuanced pitch patterns associated with different emotions and accounts for individual variability among speakers. The findings contribute to a deeper understanding of the vocal expression of emotions and offer valuable insights for advancing speech emotion recognition systems.

Recent US Congressional hearings on UAPs have made the study of exotic propulsion systems a matter of serious academic study and indeed, military necessity. This speculative paper looks at the possibility that microscopic sources could produce gravitational radiation with minimal energy requirements, whilst trapping the electromagnetic radiation stress-energy from being emitted. It may be possible to form an array of these micro-emitters of sufficient strength, acting with collective phenomenon, to allow non-linearities and the formation of soliton waves suitable for a putative warp field. Interestingly we find no need for exotic matter or negative energy. Moving onto the Alcubierre metric we ponder if the field could actually be made to travel superluminally by a repetitive stepped process of, expanding space just ahead of the craft initially to speed-up the propagation of the field, via the Shapiro Delay, so that a second field propagates that contracts space aggressively; the net effect being space contracting ahead of the craft faster than light.

Vascular endothelial growth factor (VEGF) and its receptors (VEGFRs) serve an essential role in tumor angiogenesis and have emerged as potential therapeutic targets in lung cancer. This review explores the significance of serum VEGF levels as a predictive biomarker in non-small cell lung cancer (NSCLC). The VEGF family, consisting of VEGFA, VEGFB, VEGFC, VEGFD, and placenta growth factor (PlGF), engages with specific receptors, including tyrosine kinase receptors (VEGFR-1, VEGFR-2, and VEGFR-3) and neuropilin receptors (NRP-1 and NRP-2), to promote angiogenesis and lymphangiogenesis. VEGF-A, the primary component of the VEGF family, binds to VEGFR-2 to stimulate endothelial cell proliferation and migration, while VEGF-B, VEGF-C, and VEGF-D interact with VEGFR-1 and VEGFR-3 to regulate tumor angiogenesis, lymphangiogenesis, and metastasis. The VEGF/VEGFR signaling pathway activates various downstream effectors, including phospholipase Cγ1, MAPK, and PI3K/Akt, which are essential for maintaining vascular homeostasis and promoting angiogenesis. In NSCLC, elevated serum VEGF levels have been observed, and the VEGF/VEGFR axis is frequently impaired, leading to irregular blood vessel formation and metastatic spread. Despite the development of anti-VEGF therapies, their impact on lung cancer outcomes has been limited. Further research is needed to optimize the effectiveness of these treatments and elucidate the potential of serum VEGF as a predictive biomarker in NSCLC.

Quaternion and biquaternion symmetry transformations have been applied to non-Cartesian reference systems of direct and reciprocal crystal lattices. The transformations performed directly in the sets of crystal reference axes simplify the calculations, eliminate the need for orthogonalization, permit the use of crystallographic vectors for defining the directions of rotations and perform the computations directly in the crystal coordinates. The applications of the general quaternion transformations are envisioned for physical, chemical, crystallographic and engineering applications. The general quaternion multiplication rules for any symmetry-unrestricted lattices have been derived for the triclinic crystallographic system and have been applied to the biquaternion representations of all point-group symmetry elements, including the crystallographic hexagonal system. Cayley multiplication matrices for point groups, based on the biquaternion symbols of proper and improper symmetry elements, have been exemplified.

Train fault detection primarily relies on the comparative analysis of collected images with standard reference images, making the accurate identification of the collected image types crucial for effective fault detection. Most current systems achieve indirect classification of carriage images by using Automatic Equipment Identification (AEI) devices to recognize carriage identification number information while capturing the carriage images. However, damaged RF tags or blurred characters can hinder these devices, obstructing further train fault analyses. The unique features of carriage linear array images, such as high resolution, extreme aspect ratios, and local non-linear distortions, pose significant challenges to recognition algorithms. This paper proposes a method specifically designed to recognize these types of images. First, we implement an object detection algorithm to locate the positions of key components in carriage linear array images. By leveraging the spatial layout mapping between these key components and the carriage type classification, we simplify the complex image recognition problem into a sparse point set alignment task. To ensure effective registration despite local distortions in the linear array images, we propose a weighted radial basis function. To address the issues of unknown matching relationships and quantity mismatches during point set alignment, the proposed objective function is designed to maximize the similarity between Gaussian mixtures of point sets, thereby determining the RBF weights. Extensive experiments have demonstrated that our algorithm achieves 100% recognition accuracy under local nonlinear scale distortions of less than 15%. The algorithm remains effective even in the presence of some false positives or missed detections in key component object detection results. It can accurately identify the target category from 79 image categories within 24 milliseconds on an i7 CPU without GPU support. It is of great significance to reduce system cost and promote automatic exterior fault detection for train rolling stock.

Objectives: Stress urinary incontinence (SUI) negatively affects the quality of life and sexual function in women. This study aimed to evaluate the efficacy of radiofrequency (RF) therapy in reducing SUI symptoms and its impact on sexual function. Methods: Thirty-four women with SUI were enrolled and underwent a single RF treatment session using the Viveve® System (Viveve Medical Inc., USA) with parameters of 90 J/cm² and 220 pulses per hour. Assessments at baseline and 6 months post-treatment included perineal ultrasound and personal interviews to evaluate lower urinary tract symptoms and sexual function. Urodynamic studies, voiding diaries, and questionnaires such as the Female Sexual Function Index (FSFI), Overactive Bladder Symp-tom Score (OABSS), Urogenital Distress Inventory-6 (UDI-6), Incontinence Impact Question-naire-7 (IIQ-7), and International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) measured outcomes. Results: RF therapy significantly improved sexual function, with higher FSFI scores in all domains except pain at 6 months. SUI symptoms were significantly re-duced, as indicated by improved scores on OABSS, UDI-6, IIQ-7, and ICIQ-SF, alongside better voiding diary results. Anatomical changes included reduced bladder neck mobility, decreased vaginal width, and a reduced rotation angle of the proximal urethra. Conclusions: RF therapy is effective and safe for treating mild to moderate SUI and enhances sexual function, potentially due to changes in vaginal topography. These results suggest RF therapy as a viable non-surgical op-tion for managing SUI and improving sexual health.

In the past thirty years, childhood obesity rates have risen significantly worldwide, affecting over 340 million children in affluent nations. This surge is intricately tied to metabolic disorders, notably insulin resistance, type 2 diabetes mellitus (T2DM), and the continually evolving spectrum of metabolic-associated (dysfunction) steatotic liver disease (MASLD). This review underscores the alarming escalation of childhood obesity and delves comprehensively into the evolving and dynamic changes of nomenclature surrounding diverse conditions of hepatic steatosis, from the initial recognition of non-alcoholic fatty liver disease (NAFLD) to the progressive evolution into MASLD. Moreover, it emphasizes the crucial role of pediatric endocrinologists in thoroughly and accurately investigating MASLD onset in children with T2DM where each condition influences and exacerbates the progression of the other. The review critically highlights the inadequacies of current screening strategies and diagnosis, stressing the need for a paradigm shift. A proposed solution involves the integration of hepatic magnetic resonance imaging assessment in the diagnostic arsenal for children showing insufficient glycaemic control and weight loss post-T2DM diagnosis, thereby complementing conventional liver enzyme testing. This holistic approach aims to significantly enhance diagnostic precision, fostering improved outcomes in this vulnerable high-risk pediatric population.

Introduction Image Guided Superficial Radiation Therapy (IGSRT) is a novel technology which combines traditional superficial radiation therapy (SRT) with high resolution dermal ultrasound (HRDUS) to treat non-melanoma skin cancers (NMSC). Since development, IGSRT use has expanded dramatically in outpatient clinics. We review the IGSRT literature and state of the art operating principles in US dermatologic practices. Materials and Methods A literature search of electronic databases (Medline, Pubmed, Cochrane Library, Science Direct) combined with various authors’ published and unpublished documents, procedures and clinical experience with IGSRT were synthesized for this paper. Results Studies have demonstrated IGSRT consistently delivers high cure rates( >99%) with low complications for early stage (stage 0, I, or II) squamous cell and basal cell carcinomas. Control rates are statistically superior to non-image guided SRT and external beam radiation (XRT) as well as Mohs Micrographic Surgery(MMS). This improvement is attributed to in-vivo dermal tumor visualization via HRDUS and using an interdisciplinary approach to deliver care. IGSRT use in the dermatologic clinic for early stage NMSCs has become common practice and continues to expand. Conclusion While the safety and cosmetic benefits of SRT/XRT has been long documented, IGSRT represents a significant leap forward in efficacy (statistically significant) by adding in-vivo dermal tumor imaging. Results rival and appear on one study to surpass tumor control obtained with MMS. A contributing factor to the success may be the availability and use of an interdisciplinary team approach that includes dermatologists, radiation therapists, radiation oncologists and medical physicists. The high tumor control rates, minimal side effects, favorable cosmesis and ability to treat multiple lesions per session using IGSRT is establishing this modality as a standard first line therapy for early stage NMSCs in dermatology clinics. IGSRT may represent the most effective option for the non-surgical treatment of early stage NMSC to date.

Extramammary Paget disease (EMPD) is an uncommon adenocarcinoma of apocrine gland-rich areas, presenting significant diagnostic challenges due to its nonspecific clinical appearance and frequent misidentification as benign, inflammatory skin conditions. Traditional diagnostic methods such as biopsy are invasive and uncomfortable, often required repeatedly due to high recurrence rates. Dermoscopy and non-invasive imaging techniques have been used but provide limited diagnostic accuracy due to their constraints in depth penetration and resolution. Recent advancements in imaging technologies, such as line-field confocal optical coherence tomography (LC-OCT), show promise in enhancing diagnostic precision while minimizing invasive procedures. LC-OCT merges high-resolution imaging with deep penetration capabilities, capturing detailed horizontal and vertical skin images akin to histopathology. This study evaluated the diagnostic performance of LC-OCT in detecting EMPD and its recurrence in 17 clinically suspicious anogenital regions, belonging to 6 patients. Data were collected prospectively at patient bedside by an LC-OCT expert with poor training for EMPD; and, then, reviewed retrospectively by an independent LC-OCT expert with adequate training for EMPD and no concerns about time. The prospective examination yielded 64.7%-accuracy, 71.4%-sensitivity, and 33.3%-specificity. The retrospective analysis achieved 94.1%-accuracy, 100%-sensitivity, and 66.7%-specificity, with the only false positive case being a difficult-to-diagnose concomitant presentation of a lichen sclerosus et atrophicus. Despite the need for specialized training, our results suggest that LC-OCT represents a valuable tool for accurately identifying EMPD and improve its management by reducing unnecessary biopsies. Further studies are needed to standardize its clinical application.

This article provides a detailed analysis of non-invasive techniques for prediction and diagnosis of faults in internal combustion engines, focusing on the application of the Proknow-C and Methodi Ordinatio systematic review methods. Initially, the relevance of these techniques in promoting energy sustainability and mitigating greenhouse gas emissions is discussed, aligning with the Sustainable Development Goals (SDGs) of Agenda 2030 and the Paris Agreement. The systematic review conducted in the subsequent sections offers a comprehensive mapping of the state-of-the-art, highlighting the effectiveness of combining these methods in categorizing and systematizing relevant scientific literature. The results reveal significant advancements in the use of artificial intelligence (AI) and digital signal processors (DSP) to enhance fault diagnosis, as well as underscore the crucial role of non-invasive techniques in minimizing interference in monitored systems. Finally, concluding remarks point towards future research directions, emphasizing the need to develop digital twins for internal combustion engines and identify gaps for further improvements in fault diagnosis and prediction techniques.

Seeds of Arazá and Acai, in addition to the byproduct of the Sacha inchi seed, after the extraction of the oil known as Sacha Inchi oil press-cake (SIOPC), are considered wastes and often discarded; however, they have valuable potential as sources of nutrients and bioactive compounds. We aimed to analyze, the bromatological, and chemical composition of these three byproducts of the fruit industry. Further cytotoxic, antioxidant, and antihypertensive activities of extracts ethanol: water were evaluated through in vitro tests. The Bromatological test showed that SIOPC had a low protein content (8.1%), while AS had a high fiber content (48.9%), however, were poorest in nutrients Ethanol: water extracts from all seeds exhibited diverse profiles of secondary metabolites, with significant concentrations of polyphenols (155.88 µg gallic acid/100 mg dry weight), flavonoids (411.69 µg quercetin/100 mg dry weight), and carotenoids in AS (43.66 µg ß-carotene/100 mg dry weight). AS and ACS extracts excelled in ABTS and DPPH radical scavenging assays, and AS showed high α-glucosidase inhibition, previously unreported in the literature. The results suggest that both seeds and extracts are sources of nutritional and bioactive metabolites with high antioxidant and hypoglycemic activity. that could be used in the food industry and the health sector. These by-products underline the importance of sustainable harnessing these sources of the native Amazonian Region

Pollution, urbanization and technological advancements have posed stress on the ecosystem and its matrices. Globally, pollutants are observed in all compartments of ecological communities which affect the integrity of the atmosphere, hydrosphere, lithosphere and biosphere. Xenobiotics bioaccumulate and biomagnify across trophic levels or webs. Heavy metals have been implicated in impacts towards biodiversity and environmental declination. In Nigeria, wastes issues are a cause of concern and landfilling or are waste disposal approaches that cause deleterious effects such as depauperation of diversity and species decline, prompting the non-invasive ecotoxicological hazard assessments. Feathers were used to assess concentration levels of 13 metals (Fe, Mn, Zn, Cu, Co, Cr, Cd, Pb, Ni, Al, B, Se and Hg) using fast atomic absorption spectrophotometer (FAAS), one-way ANOVA was used in comparing variations in mean concentrations across trophic levels, diversity indices and point count methods were used to estimate diversity and abundance. Four avian species were assessed Hirundo aethiopica (insectivore), Streptoptelia senegalensis (granivore), Turdus pelios (omnivore), and Anthus leucophrys. (insectivore). Observed concentrations of important metals like iron were high with the insectivores having the highest concentrations of iron (2.6985 ± 0.1975 ppm) and granivores having the lowest (2.0100 ± 0.3172 ppm). The insectivores had the highest concentrations for cadmium, cobalt and chromium (0.0005 ± 0.0005ppm), nickel (at 00.0012 ± 0.0017 ppm) and least was observed in the granivores (0.0003 ± 0.0006 ppm). Selenium concentrations were quite high in insectivores (0.0775 ± 0.0035 ppm) and lowest in granivores (0.0433 ± 0.0015 ppm) likewise mercury for insectivores (0.0280 ± 0.0010 ppm) and granivores (0.0213 ± 0.0015 ppm). From the study, it is apparent that landfills are sources of pollution and poor management of wastes can affect biota (causing effects such as asymmetry and bioaccumulation) and ecosystem balance. The observation of heavy metals in the feathers serve as good sentinels due to their trophic levels and could be signs for what could be yet to occur. The heavy metal concentrations were significantly different at p < 0.05.

This paper presents an extensive performance analysis of open-ended waveguide elements for direct radiating arrays with high scan angle (± 50º/60º). The evaluated designs are based on square and hexagonal apertures loaded with ridges. Both square and triangular lattices are considered in the framework of Ka-band downlink design requirements for future LEO mega-constellations. The parameter space defined by the monomodal condition has been explored to find an optimum value for each structure. The analyses are validated with both infinite and finite full-wave models in terms of active reflection coefficient, scan loss and cross-polar discrimination.

Abstract: Palynomorph assemblages and mineral microfossils from five southern Red Sea coastal environments were studied to demonstrate their distribution pattern and suggest their source. The environments studied were 1. mangrove swamp 2. middle Holocene paleochannel 3. algal mat, 4. Sabia Island coral reef, and intertidal zone. The samples studied were from the top 3 cm semi-consolidated sediments from various environments. Standard palynological maceration technique was used to isolate palynomorphs. These sediment samples are mainly clay and fine sand and yielded low numbers but high diversity of palynomorph assemblages. They are grouped into pollen and spores, dinoflagellate cysts, and algal remains, fungal spores, hyphae and fruit bodies, protists, and invertebrates. A few Ascidian and sponge spicules, and phytoliths (mineral microfossils) occur in the Sabia Island coral reef environment. The protists and invertebrate remain include microforaminifera, thecamoebians, tintinnomorphs, crustacean and annelid palynomorphs. The assemblage also includes six form taxa belonging to the form genera Katora and Mangrovia. These palynomorphs belong to both marine and terrestrial environments and are of autochthonous and allochthonous origins. The source and genetic relationship of each palynomorphs is discussed.

Androgenetic alopecia, the most common cause of hair loss affecting both men and women, is typically treated using pharmaceutical options such as minoxidil and finasteride. While these medications work for many individuals, they are not suitable options for all. To date, the only non-pharmaceutical option that the United States Food and Drug Administration has cleared as a treatment for androgenetic alopecia is low-level laser therapy (LLLT). Various clinical trials utilizing LLLT devices of various types are available. However, a myriad of other physical treatments for this form of hair loss are reported in the literature. This review evaluated the effectiveness of microneedling, pulsed-electromagnetic field (PEMF) therapy, low-level laser therapy (LLLT), fractional laser therapy, and non-ablative laser therapy for the treatment of androgenetic alopecia (AGA). It also explores the potential of multimodal treatments combining these physical therapies. LLLT has the majority of evidence in the literature as a physical therapy for androgenetic alopecia. However, other physical treatments, such as non-ablative laser treatments, and multimodal approaches, such as PEMF-LLLT, seem to have the potential to be equally or more promising and merit further exploration.

Sepsis still remains one of the most difficult conditions to diagnose as it presents unspecific clinical signs and symptoms, which makes early detection crucial. Our study aimed to enhance the accuracy of sepsis diagnosis by integrating multimodal monitoring technologies with traditional diagnostic methods. The research involved 121 newborns, comprising 39 cases of late-onset sepsis, 35 cases of early-onset sepsis, and 47 control subjects. Biosignals including pulse oximetry (PO), near-infrared spectroscopy (NIRS), and skin temperature (ST) were continuously monitored and an algorithm was developed using Python for identifying early signs of sepsis. The model demonstrated the ability to detect sepsis 6 to 48 hours in advance with an accuracy rate of 87.67% ± 7.42%. Sensitivity and specificity were recorded at 76% and 90%, respectively, with NIRS and ST having the most significant impact on predictive accuracy. Despite the promising results, limitations such as sample size, data variability, and potential biases were noted. The findings highlight the critical role of non-invasive biosensing methods in conjunction with traditional biomarkers and cultures, offering a robust framework for early sepsis detection and improved neonatal care. Further studies are recommended to validate these results across diverse clinical settings.

In this paper, the time-resolved model of photoacoustic signal for samples with a complex inner structure is derived including local non-equilibrium of structural elements with multiple degrees of freedom, i.e. structural entropy of the system. The local non-equilibrium is taken into account through the fractional operator. By analyzing the model for two types of time-dependent excitation, a very short pulse and a very long pulse, it is shown that the rates of non-equilibrium relaxations in complex samples can be measured by applying the derived model and time-domain measurements. Limitations of the model and further directions of its development are discussed.

Cutaneous squamous cell carcinoma is a prevalent malignancy with a rising incidence and notably high mutational load. Exploring the genetic nuances of cSCC and investigating molecular approaches stands as a potential avenue for improving outcome in high-risk patients. This retrospective case-control study involved two cohorts of 14 patients (“discovery cohort”) and 12 patients (“validation cohort”) with cSCC located in head-neck anatomical district and diagnosed at pT2 stage. Overall, cases developed early local relapse of the disease whereas controls never re-lapsed during the entire follow-up period. Next generation sequencing (NGS) approach conducted on histological samples revealed that TP53 and CDKN2A were the most frequently mutated genes in our series. No specific mutation was identified as potential prognostic or therapeutic targets. Controls exhibited a tendency toward a higher mutational rate compared to cases. It is possible that an increased number of mutations could prompt the cSCC to expose more antigens, becoming more immunogenic and facilitating the recognition by the immune system. This could enhance and sustain immunological response, potentially preventing future recurrences.

Predators are dependent on the capture of prey to meet their energetic and nutritive requirements, imposing a regime of predation risk. Consequently, prey experience both consumptive and non-consumptive effects. Non-consumptive effects may manifest through altered growth and ontogenetic trajectories of prey species, a dynamic modulated by olfactory or other sensory cues from predators. Bactrocera dorsalis Hendel represents a great invasive threat to global horticulture. While earlier research has primarily centered on the consumptive interactions between B. dorsalis and its natural enemies, the potential consequences of non-consumptive interactions on the development of B. dorsalis have been overlooked. In this study, we investigated the impact of predation risk effects, induced by both visual exposure to the predatory mantis Hierodula patellifera Serville and its associated odor, on the life history traits of B. dorsalis. Female B. dorsalis demonstrated reduced development time in the presence of isolated H. patellifera or H. patellifera odor, yet evidenced significantly increased fecundity. Conversely, males displayed no significant change in development time. Additionally, neither female nor male body weight at death was significantly influenced by predation risk from isolated predators or predator odor. These results indicated adaptive developmental strategies of B. dorsalis when faced with risks from predators and highlight the important role of predator odor, which has the potential to lay the groundwork for improving biological control efforts and innovative approaches within pest control.
